胃复春联合HP根除术治疗HP相关萎缩性胃炎有效性和安全性的Meta分析

摘　　要：目的:从循证医学角度探讨胃复春联合HP根除术治疗HP相关萎缩性胃炎患者的有效率。方法:计算机检索中国知网、万方数据库、维普网、中国生物医学文献数据库、The cochrane library、PubMed、Embase数据库中筛选关于胃复春联合HP根除术治疗HP相关萎缩性胃炎的随机对照实验,检索时间至2021年12月。由两名研究者分别筛选文献、提取一般文献特征和数据,并使用相关评价量表对所选文献进行质量评价,采用RevMan5.3对其进行Meta分析和异质性检验。结果:最终13篇文献符合纳入标准。经Meta分析得出,萎缩性胃炎伴HP阳性患者,相比较单独使用HP根除术者,使用胃复春联合HP根除术治疗能提高临床疗效(OR=1.18,95%CI=1.11-1.24,P<0.00001)、提高HP清除率(OR=1.23,95%CI=1.10-1.37,P<0.001)、降低不良反应发生率(OR=0.45,95%CI=0.25-0.82,P<0.01)。经敏感性分析得出,纳入的研究Meta分析结果稳定。结论:在HP根除术基础上加用胃复春可提高HP相关CAG患者临床疗效、提高HP清除率、降低不良反应发生率,提高治疗有效性和安全性。Objective To explore the efficacy of weifuchun combined with HP eradication in the treatment of HP-related atrophic gastritis from the perspective of evidence-based medicine.Methods Randomized controlled trials related to weifuchun eradication in The treatment of HP-related atrophic gastritis were searched from CNKI,Wanfang Database,VIP,Chinese Biomedical Literature Database,The Cochrane Library,PubMed and Embase databases until December 2021.Two researchers screened the literature,extracted general literature features and data,and used relevant evaluation scales to evaluate the quality of the selected literature.Meta-analysis and heterogeneity test were conducted by RevMan5.3.Results 13 references met the inclusion criteria.Meta-analysis showed that weifuchun combined with HP eradication could improve clinical efficacy(OR=1.18,95%CI=1.11-1.24,P<0.00001)and HP clearance rate(OR=1.23,95%CI=1.10-1.37,P<0.001)for atrophic gastritis patients with HP positive,compared with HP eradication alone,and reduced the incidence of adverse reactions(OR=0.45,95%CI=0.25-0.82,P<0.01).After sensitivity analysis,meta-analysis results of the included studies were stable.Conclusion The addition of weifuchun on the basis of HP eradication can improve the clinical efficacy,improve the HP clearance rate,reduce the incidence of adverse reactions,and improve the efficacy and safety of treatment for patients with HP related CAG.
