摘  要:目的研究Shaker训练联合生物反馈对脑梗死后吞咽障碍患者的影响。方法选取2020年1月~2022年1月新疆维吾尔自治区人民医院收治的188例脑梗死后吞咽障碍患者,按照随机数表法分为两组,各94例。其中对照组进行常规吞咽障碍训练,观察组在常规训练基础上实施Shaker训练联合生物反馈训练。比较两组治疗疗效,观察两组治疗前及治疗4、8周后视频荧光吞咽摄影检查(VFSS)评级及标准吞咽功能评定法(SSA)评分,并分析两组表面肌电(sEMG)检查情况。结果观察组治疗有效率为73.40%,显著高于对照组的51.06%,差异有统计学意义(χ^(2)=8.266,P=0.004)。两组治疗8周时VFSS分级及SAA评分均显著低于治疗前及治疗4周,治疗4周时VFSS分级及SAA评分均显著低于治疗前,且观察组治疗4、8周时VFSS分级及SAA评分均显著低于对照组,差异有统计学意义(t=4.549、4.042、5.292、5.520,P<0.01)。两组治疗后sEMG平均波幅均显著高于治疗前,sEMG吞咽时限显著低于治疗前,且观察组治疗后sEMG平均波幅均显著高于对照组,s EMG吞咽时限显著低于对照组,差异有统计学意义(P<0.01)。结论Shaker训练联合肌电生物反馈训练能有效锻炼脑梗死后吞咽障碍患者吞咽肌群,提升吞咽功能,具有较好的临床应用价值。Objective To study the effect of shaker training combined with biofeedback on patients with dysphagia after cerebral infarction.Methods 188 patients with dysphagia after cerebral infarction treated in our hospital from January 2020 to January 2022 were included as the research objects.They were randomly divided into two groups with 94 cases in each group.The control group received routine dysphagia training,and the observation group received shaker training combined with biofeedback training on the basis of routine treatment.The therapeutic effects of the two groups were compared.The ratings of VFSS and SSA were observed before and 4 and 8 weeks after treatment,and the sEMG of the two groups were analyzed.Results The effective rate of the observation group was 73.40%,which was significantly higher than 51.06%of the control group(χ^(2)=8.266,P=0.004).The VFSS grade and SAA score of the two groups at 8 weeks of treatment were significantly lower than those before treatment and 4 weeks of treatment,and the VFSS grade and SAA score at 4 weeks of treatment were significantly lower than those before treatment;The VFSS grade and SAA score of the observation group were significantly lower than those of the control group at 4 and 8 weeks of treatment(t=4.549、4.042、5.292、5.520,P<0.01).The average amplitude of sEMG after treatment was significantly higher than that before treatment,and the swallowing time limit of sEMG was significantly lower than that before treatment;After treatment,the average amplitude of sEMG in the observation group was significantly higher than that in the control group,and the swallowing time of s EMG was significantly lower than that in the control group(P<0.01).Conclusion Shaker training combined with EMG biofeedback training can effectively exercise the swallowing muscles of patients with dysphagia after cerebral infarction and improve the swallowing function.It has good clinical application value.
