多层螺旋CT征象与肿瘤标志物水平对BorrmannⅠ型胃癌的诊断价值研究

摘　　要：目的分析多层螺旋CT与血清癌胚抗原(carcinoembryonic antigen,CEA)、胃肠道癌抗原242(Gastrointestinal cancer antigens 242,CA242)对BorrmannⅠ型胃癌的诊断价值。方法选取2017年3月~2021年12月于淮南东方医院集团总医院就诊的80例BorrmannⅠ型胃癌患者为胃癌组,同期胃炎患者50例为对照组,比较CT、CEA、CA242单独检测及三者联合诊断胃癌的诊断价值。结果胃癌组CT征象见胃壁增厚>10 mm、胃腔内肿块、胃部溃烂、浆膜层模糊、基底增宽;肿瘤最大直径、肿瘤厚度与浸润深度和淋巴结转移密切相关(P<0.05),与远处转移无关(P>0.05);胃癌组血清CEA、CA242水平高于对照组,差异有统计学意义(P<0.05);三项联合胃癌检出率高于CT、CEA、CA242单项检测检出率,差异有统计学意义(P<0.05)。结论对于BorrmannⅠ型胃癌,多层螺旋CT、肿瘤标志物CEA、CA242联合应用可提高检出率。Objective To analyze the diagnostic value of multi-slice spiral CT,serum carcinoembryonic antigen(CEA)and gastrointestinal cancer antigen 242(CA242)in Borrmann typeⅠgastric cancer.Methods from March 2017 to December 2021,80 patients with Borrmann typeⅠgastric cancer were treated in our hospital as gastric cancer group,and 50 patients with gastritis in the same period were treated as control group.The diagnostic value of CT,CEA,CA242 alone and the combination of the three were compared.Results the CT signs of gastric cancer group showed thickening of gastric wall>10mm,mass in gastric cavity,gastric ulceration,blurred serosa layer and widened base;The maximum diameter and thickness of tumor were closely related to the depth of invasion and lymph node metastasis(P<0.05),but not to distant metastasis(P>0.05);The level of serum CEA and CA242 in gastric cancer group was higher than that in control group(P<0.05);The detection rate of three combined gastric cancer was higher than that of CT,CEA and CA242(P<0.05).Conclusion For Borrmann typeⅠgastric cancer,the combination of multi-slice spiral CT,tumor markers CEA and CA242 can improve the detection rate.
