原发性肝癌经导管肝动脉化疗栓塞术后不同碘油沉积患者术前影像特征比较

摘　　要：目的比较原发性肝癌经导管肝动脉化疗栓塞术(TACE)术后不同碘油沉积组术前CT/磁共振成像(MR)影像学特征。方法根据TACE术后碘油沉积情况,将62个病例共71个病灶分为碘油沉积良好组、碘油沉积不佳组,比较其在术前病灶大小、动脉期是否强化、是否“快进快出样”强化、有无假包膜、有无门静脉癌栓方面差异。结果碘油沉积不佳组,病灶平均直径(5.8±4.0)cm,动脉期强化23例,快进快出17例,假包膜13例,门静脉癌栓10例;碘油沉积良好组,病灶平均直径(3.7±3.2)cm,动脉期强化25例,快进快出18例,假包膜19例,门静脉癌栓12例。结论2组病例在病灶大小、动脉期强化、假包膜差异有统计学意义,“快进快出”样强化、门静脉癌栓差异无统计学意义。Objective To compare the preoperative computed tomography(CT)/magnetic resonance(MR) imaging characteristics of different iodine oil deposition groups after transarterial chemoembolization(TACE) for primary liver cancer. Methods Based on the iodine oil deposition after TACE, a total of 71 lesions in 62 patients were divided into the good iodine oil deposition and poor iodine oil deposition groups. Statistical differences in preoperative lesion size, arterial phase enhancement, presence of fast-in fast-out-like enhancement, presence of a pseudocapsule tumor envelope, and presence of portal vein tumor thrombosis were compared between the groups. Results In the poor iodine deposition group, the mean diameter of the lesions was(5.8 ±4.0)cm, with 23 cases with arterial phase enhancement, 17 cases fast-in and fast-out enhancement, 13 cases pseudocapsule envelopment, and 10cases portal vein tumor thrombosis. In the good iodine deposition group, the mean diameter of the lesions was(3.7 ±3.2)cm, with 25 cases arterial phase enhancement, 18 cases fast-in and fast-out enhancement, 19 cases pseudocapsule envelopment, and 12 cases portal vein tumor thrombosis. Conclusion Lesion size, arterial phase enhancement and pseudocapsule envelopment significantly differed between the groups, whereas no significant differences were found in fast-in fast-out-like enhancement and portal vein tumor thrombosis.
