水平斜视术后屈光状态和角膜地形图的临床研究

摘　　要：目的探究水平斜视术后眼球屈光状态和角膜地形图的变化。方法收集2018年1月至2018年12月在我院行水平显微斜视矫正术并坚持随访的患者共56例79眼,按不同手术方式分组;比较术前1天与术后1周、1月及3月的等效球镜、球镜、柱镜、散光轴位及角膜平均曲率。结果术后1周较术前等效球镜屈光度和柱镜屈光度的改变有统计学意义(P<0.05),而术后3月较术前无统计学意义(P>0.05)。术后1周和术后1月散光眼比例增加(P<0.05),其中以顺规散光增加为主;术后3月和术前散光构成比差异无统计学意义(P>0.05)。角膜地形图曲率的改变较术前无统计学意义(P>0.05)。结论术后等效球镜和柱镜变化明显,术后散光轴位改变以顺规散光增加为主。水平斜视术后屈光状态的改变与角膜曲率无关。Objective To investigate the changes of eye refractive status and corneal topography after horizontal strabismus.Methods A total of 56 patients with 79 eyes who underwent horizontal microscopic strabismus correction in our hospital from January to December 2018 and adhered to follow-up were collected and grouped according to different surgical methods;Equivalent spherical lens,spherical lens,cylinder lens,axial astigmatism and mean corneal curvature were compared between 1 day before surgery and 1 week,1 month and 3 months.Results The changes of equivalent spherical diopter and cylinder diopter were statistically significant at 1 week after operation(P<0.05),but not at 3 months after operation(P>0.05).The proportion of astigmatism increased at 1 week and 1 month after operation(P<0.05),among which the increase of compliant astigmatism was the main one;there was no significant difference in the astigmatism composition ratio between 3 months after operation and before operation(P>0.05).The change of corneal topography curvature was not statistically significant compared with that before operation(P>0.05).Conclusion Postoperative equivalent spherical and cylindrical changes are obvious;postoperative astigmatism axis changes are mainly the increase of compliant astigmatism.Changes in refractive status after horizontal strabismus were independent of corneal curvature.
