摘 要:目的 探究大蒜素对呼吸道感染患者多药耐药肺炎克雷伯菌毒力基因表达的影响。方法 收集25株呼吸道感染患者多药耐药肺炎克雷伯菌进行药敏实验;采用实时荧光定量聚合酶链式反应(PCR)检测耐药株、敏感株中耐药基因超广谱β-内酰胺酶(ESBLs)的携带率。用不同浓度大蒜素检测多药耐药肺炎克雷伯菌抑菌环的效果;用时间-杀菌曲线检测大蒜素对多药耐药肺炎克雷伯菌杀菌效果;采用实时荧光定量PCR检测大蒜素对多药耐药肺炎克雷伯菌毒力基因和耐药基因的表达水平。结果 25株肺炎克雷伯菌对环丙沙星、左氧氟沙星、磺胺甲噁唑/甲氧苄啶、氨苄西林、头孢吡肟、阿莫西林耐药率>60%。耐药株内耐药基因ESBLs的携带率高于敏感株。25μg/ml大蒜素组、50μg/ml大蒜素组、100μg/ml大蒜素组中多药耐药肺炎克雷伯菌的抑菌环直径高于NC组;随着时间增加,25μg/ml大蒜素组、50μg/ml大蒜素组、100μg/ml大蒜素组对多药耐药肺炎克雷伯菌有更强的抑制作用。25μg/ml大蒜素组、50μg/ml大蒜素组、100μg/ml大蒜素组中多药耐药肺炎克雷伯菌毒力相关基因iucA、iroB、iutA、wcaG低于NC组,耐药基因ESBLs的携带率低于NC组。结论 大蒜素与抗菌药物使用对呼吸道感染患者多药耐药肺炎克雷伯菌具有良好的杀菌作用,还能够降低毒力基因iucA、iroB、iutA、wcaG表达水平及耐药基因ESBLs的携带率。OBJECTIVE To explore the effect of allicin on the expression of virulence genes of multi-drug resistant Klebsiella pneumoniae in patients with respiratory tract infections. METHODS Twenty-five strains of multi-drug resistant K. pneumoniae from patients with respiratory tract infections were collected for drug susceptibility experiments. Real-time fluorescent quantitative polymerase chain reaction(PCR) was used to detect the expression levels of resistant genes of ESBLs in drug-resistant and susceptible strains. Different concentrations of allicin were used to detect the antibacterial effect of multidrug-resistant K. pneumoniae;the time-kill curve was used to detect the bactericidal effect of allicin on multidrug-resistant K. pneumonia;the expression levels of allicin′s virulence genes and resistance genes against multidrug-resistant K. pneumoniae were detected by real-time fluorescent quantitative PCR. RESULTS The drug-resistant rates of 25 strains of K. pneumoniae to ciprofloxacin, levofloxacin, compound sulfamethoxazole, ampicillin, cefepime, and amoxicillin, exceeded 60%. The carrying rates of ESBLs genes in the drug-resistant strains were higher than those in the drug-susceptible strains. The diameter of antibacterial ring of multidrug-resistant K. pneumoniae in the 25 μg/ml allicin group, 50 μg/ml allicin group and 100 μg/ml allicin group were higher than that in of the NC group. With the increase of time, the bactericidal effect of allicin on multidrug-resistant K. pneumoniae among 25 μg/ml allicin group, 50 μg/ml allicin group, and 100 μg/ml allicin group was inhibited more remarkably. The expression levels of virulence-related genes iucA, iroB, iutA, and wcaG of multidrug-resistant K. pneumoniae in the 25 μg/ml allicin group, 50 μg/ml allicin group, and 100 μg/ml allicin group were higher than those in the NC group, indicating the carrying rate of resistance gene ESBLs in drug-resistant and susceptible strains was higher than that in the NC group.
