摘 要:目的探讨高危型人乳头瘤病毒感染后宫颈癌前病变(CIN)与抗原处理相关转运蛋白1/2(TAP1/2)基因多态性的关联性。方法选择医院妇科2018年5月-2021年5月收治高危型HPV感染CIN患者65例为研究组。选择同期医院尚未开始治疗的高危型HPV感染子宫颈炎患者65例为对照组,两组均在入院当天抽取外周血2 ml加入乙二胺四乙酸抗凝采血管中,采用聚合酶链式反应(PCR)扩增TAP1、TAP2基因多态性目的片段。入院时通过病历系统收集入组对象年龄、分娩次数、体质量指数、HPV型别、HPV亚型数量。统计分析研究组宫颈癌患者临床病理资料。结果研究组与对照组TAP1 rs41549617位点基因型整体分布有统计学差异(P<0.05);研究组TAP1 rs41549617位点AA基因型、A等位基因频率高于对照组(P<0.05);研究组CIN患者携带TAP1 rs41549617携带AA型者CIN等级高于携带GG/GA型者(P<0.05);研究组CIN患者携带TAP2 rs1135216位点携带AA型者累及腺体发生率高于携带GG/GA型者(P<0.05)。结论TAP1 rs41549617位点影响高危型HPV感染者发展为CIN,且与CIN等级有关,TAP2 rs1135216位点与CIN易感性无关,但影响累及腺体。OBJECTIVE To investigate the association between cervical precancerous lesions(CIN)and gene polymorphisms of antigen processing related transporter 1/2(TAP1/2)in patients after high-risk human papillomavirus infection.METHODS Total of 65 cervical cancer patients with high-risk HPV infection admitted to our hospital from May 2018 to May 2021 were recruited as the study group.Another 65 patients with high-risk HPV infection-induced cervicitis who had not yet been treated in the hospital during the same period were enrolled as the control group.On the day of admission,2 ml of peripheral blood was collected into EDTA anticoagulant blood collection vessel.The polymorphism target fragments of TAP1 and TAP2 genes were amplified by polymerase chain reaction(PCR).Data such as age,delivery times,body mass index,HPV type and HPV subtype number of the subjects in the group were collected through the medical record system at admission.The clinical and pathological data of cervical cancer patients in the study group were statistically analyzed.RESULTS TAP1 the overall distribution of genotypes at rs41549617 locus between the study group and the control group was significantly different(P<0.05).The frequency of AA genotype and A allele at TAP1 rs41549617 locus in the study group was higher than that in the control group(P<0.05).In the study group,CIN grade in patients carrying AA genotype at TAP1 rs41549617 was higher than those carrying GG/GA genotype(P<0.05).The incidence of gland involvement in the study group CIN patients carrying AA genotype at TAP2 rs1135216 locus was higher than those carrying GG/GA genotype(P<0.05).CONCLUSION TAP1 rs41549617 locus affects the development of CIN in high-risk HPV infections,which is related to CIN grades.TAP2 rs1135216 locus is not related to the susceptibility of CIN,but affects glandular involvement.
