摘 要:目的分析细菌感染及子宫内膜组织中核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)炎症小体-白细胞介素(IL)-1β表达在子宫内膜癌病变中的临床意义。方法选择2015年1月-2020年1月于天津市第五中心医院接受手术治疗的93例子宫内膜癌患者(研究组),另选择同期接受手术治疗的60例子宫肌瘤患者(对照组),比较两组患者子宫内膜组织及宫颈分泌物细菌培养阳性率,采用免疫组化法检测子宫内膜组织中NLRP3、IL-1β表达,采用实时荧光定量聚合酶链式反应法检测子宫内膜组织中NLRP3、凋亡相关斑点样蛋白(ASC)、半胱氨酸蛋白酶(Caspase-)1、IL-1β和IL-18 mRNA表达,并比较子宫内膜癌病变患者不同临床病理特征组织中NLRP3、IL-1β表达。结果研究组子宫内膜组织及宫颈分泌物细菌培养阳性率(31.18%和43.01%)高于对照组(3.33%和11.67%)(P<0.05);研究组子宫内膜组织中NLRP3、IL-1β阳性表达率高于对照组(P<0.05),NLRP3、ASC、caspase-1、IL-1β及IL-18 mRNA表达高于对照组(P<0.05);NLRP3在子宫内膜癌病变组织中的表达与肿瘤FIGO分期、分化类型有关(P<0.05),IL-1β在子宫内膜癌病变组织中的表达与肿瘤FIGO分期、分化类型及淋巴结转移有关(P<0.05)。结论子宫内膜癌病变患者细菌感染率上升,子宫内膜组织中NLRP3及ASC、caspase-1、IL-1β、IL-18表达上调,NLRP3可能通过组装成炎症小体在子宫内膜癌病变中发挥重要作用。OBJECTIVE To analyze the clinical significance of bacterial infection and expression of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)inflammasome/interleukin(IL)-1βsignaling pathway in endometrial tissues from endometrial carcinoma patints.METHODS Total of 93 patients with endometrial carcinoma who underwent surgical treatment in Tianjin Fifth Central Hospital between Jan 2015 and Jan 2020 were recruited as the study group,and 60 patients with uterine fibroids receiving surgical treatment during the same period were enrolled as the control group.The positive rates of bacterial culture in endometrial tissues and cervical secretions were compared between the two groups.Immunohistochemistry was used to detect the expression levels of NLRP3 and IL-1βin endometrial tissues.Quantitative real-time polymerase chain reaction(qRT-PCR)was applied to measure the mRNA expression of NLRP3,apoptosis-associated speck-like protein containing a CARD(ASC),cysteine protease(caspase-1),IL-1βand IL-18 in endometrial tissues.The expression of NLRP3 and IL-1βin tissues among endometrial carcinoma patients with different clinicopathological characteristics were compared.RESULTS The positive rates of bacterial culture in endometrial tissues and cervical secretions in study group were 31.18%and 43.01%,respectively,which were higher than those of 3.33%and 11.67%in the control group(P<0.05).The positive expression rates of NLRP3 and IL-1βin endometrial tissues of the study group were higher than those of the control group(P<0.05);the mRNA expression levels of NLRP3,ASC,caspase-1,IL-1βand IL-18 were higher than those in the control group(P<0.05).The expression of NLRP3 in lesion tissues of endometrial carcinoma was related to the tumor FIGO staging and differentiation type(P<0.05),and the expression of IL-1βin lesion tissues of endometrial carcinoma was correlated with the tumor FIGO staging,differentiation type and lymph node metastasis(P<0.05).CONCLUSION The bacterial infection rates of patients with endome
