血清外泌体miR-17-92a表达水平与行结直肠癌根治术中注射5-氟尿嘧啶治疗的结直肠癌患者预后的关系

摘　　要：目的探讨血清外泌体miR-17-92a表达水平与结直肠癌根治术中注射5-氟尿嘧啶(5-Fu)的结直肠癌患者预后的关系。方法选择93例行结直肠癌根治术且术中注射5-Fu治疗的结直肠癌患者作为观察组,选择同期50例健康体检者作为对照组。比较两组的血清外泌体miR-17-92a表达水平,以及不同临床病理特征的结直肠癌患者的血清外泌体miR-17-92a表达水平。术后随访3年,记录不同血清外泌体miR-17-92a表达水平患者的生存情况。结果观察组患者血清外泌体miR-17-92a表达水平高于对照组(P<0.05)。相比于TNM分期为Ⅰ~Ⅱ期、无淋巴结转移、无远处转移的结直肠癌患者,TNM分期为Ⅲ~Ⅳ期、存在淋巴结转移、远处转移的患者的血清外泌体miR-17-92a表达水平更高(均P<0.05)。血清外泌体miR-17-92a表达阳性的结直肠癌患者术后中位生存期短于表达阴性者(P<0.05)。结论结直肠癌患者血清外泌体miR-17-92a表达水平异常升高,血清外泌体miR-17-92a表达阳性患者接受结直肠癌根治术联合术中注射5-Fu治疗后的预后较差。Objective To explore the relation of serum exosome miR-17-92a with prognosis of patients with colorectal cancer undergoing injection of 5-fluorouracil(5-Fu)during radical resection of colorectal cancer.Methods A total of 93 patients with colorectal cancer who underwent injection of 5-Fu therapy during radical resection of colorectal cancer were selected as observation group,and 50 healthy check-up people during the same period were selected as control group.The expression of serum exosome miR-17-92a between the two groups,as well as expression of serum exosome miR-17-92a between the patients with colorectal cancer in different clinicophathological features were compared.After a 3-year follow-up of operation,the survival conditions of patients with different expressions of serum exosome miR-17-92a were recorded.Results The patients in the observation group obtained higher expression of serum exosome miR-17-92a as compared with the control group(P<0.05).Compared to colorectal cancer patients withⅠ-Ⅱstages of TNM staging,without metastasis of lymph nodes,and without distant metastasis,the expression of serum exosome miR-17-92a was higher in patients withⅢ-Ⅳstages of TNM staging,presence of lymph nodes metastasis and distant metastasis(all P<0.05).The postoperative median survival time of colorectal cancer patients with positive expression of serum exosome miR-17-92a was shorter than that of colorectal cancer patients with negative expression(P<0.05).Conclusion The expression of serum exosome miR-17-92a in patients with colorectal cancer is abnormally elevated,and the prognosis of colorectal cancer patients with positive expression of serum exosome miR-17-92a undergoing injection of 5-Fu therapy during radical resection of colorectal cancer is relatively poor.
