摘　　要：目的本研究拟探讨下颌前移口腔矫治器(OA)治疗阻塞性睡眠呼吸暂停与失眠共病(COMISA)的疗效。方法选取2013年4月至2019年8月至广西壮族自治区人民医院就诊的14例COMISA患者作为共病组、17例单纯鼾症患者作为对照组,对共病组OA治疗前、治疗1个月后及对照组治疗前的多导睡眠图(PSG)检查参数进行比较。结果共病组治疗后,睡眠潜伏期(SL)、觉醒指数(ArI)、暂停指数(AI)、呼吸暂停低通气指数(AHI)减少(P<0.05),总睡眠时间(TST)、睡眠效率(SE)、N3期及最低血氧饱和度(LSpO_(2))增加(P<0.05)。与对照组相比,共病组治疗前SL、ArI、N1期百分比较大,TST、SE、N2期百分比较小,治疗后快速眼球运动睡眠潜伏期、ArI较大,TST、SE较小(P<0.05)。结论OA是治疗COMISA的较好疗法。Objective This study aims to investigate the efficacy of mandibular advancement oral appliance(OA)for COMISA.Methods 14 patients with COMISA were selected as a comorbid group and 17 with primary snoring as a control group.All the patients were treated in the People’s Hospital of Guangxi Zhang Autonomous Region from April 2013 to August 2019.The polysomnography(PSG)parameters in the comorbid group before OA treatment and one month after treatment and those in the control group to treatment were compared.Results After OA treatment,sleep latency(SL),arousal index(ArI),apnea index(AI),apnea hypopnea index(AHI)decreased(P<0.05),while total sleep time(TST),sleep efficiency(SE),N3 phase and lowest oxygen saturation(LSpO_(2))increased in the comorbid group(P<0.05).As compared with the control group,the percentage of SL,ArI,N1,TST,SE and N2 was larger in the comorbid group before treatment,but the percentage of TST,SE and N2 was smaller.After treatment,REM sleep latency and ArI were larger,and TST and SE were smaller in the comorbid group(P<0.05).Conclusions Oral appliance is a better therapy for COMISA.
