基于“情志学说”探讨双心疾病的中医辨治

摘　　要：基于情志学说探讨双心疾病的病因病机,提出相应的治则治法,遣方用药,并结合中医特色心理疗法以论治本病。双心疾病病位在心,与肝、脾、肾密切相关。病性总属本虚标实,虚为气血阴阳不足,心神失养;实为气滞、血瘀、寒凝、痰浊。病机为肝郁气滞、痰瘀互结、心脾两虚和心肾不交。双心疾病辨治思路为:双心同治,重视疏肝;调和营卫,顾护心体;通补兼施,并调兼证;平调阴阳,以和为期。同时,辅以情志相胜法、情志调摄法、移情变气法、五行音乐法、开导劝说法等中医心理疗法以及导引运动、针灸推拿等中医特色疗法,对提升双心疾病临床疗效具有重要意义。This paper tries to analyze the etiology and pathogenesis of double heart disease from the emotional theory of traditional Chinese medicine,put forward the corresponding treatment principles and methods,send prescriptions and drugs,and combine the characteristic psychotherapy of traditional Chinese medicine to treat this disease.Double heart disease is located in the heart and is closely related to the liver,spleen and kidney.The general administration of disease nature is deficiency in origin and excess in superfcislity.The deficiency includes lack of Qi,blood,Yin and Yang,as well as the loss of mental health.The excess includes Qi stagnation,blood stasis,cold coagulation and phlegm turbidity.The pathogenesis is liver depression and Qi stagnation,phlegm and blood stasis,deficiency of the heart and spleen,and heart and kidney failure.The thought of differentiation and treatment of double heart disease is to treat both hearts together,pay attention to soothing the liver,coordinate the camp and guard,take care of the heart and body,apply both tonifying and complementing,adjust and prove simultaneously,and balance Yin and Yang to make harmony.At the same time,traditional Chinese medicine psychotherapy such as the method of winning emotions,the method of adjusting emotions,the method of changing emotions into Qi,the method of five elements music,the method of enlightening and persuading,as well as the treatment of traditional Chinese medicine characteristic therapies such as guiding exercise,acupuncture and massage are of great significance to improve the clinical efficacy of double heart disease.
