摘  要:目的探讨磁共振扩散峰度成像(DKI)参数评估鼻咽癌裸鼠移植瘤乏氧和放疗疗效的应用价值。材料与方法构建裸鼠CNE-1(放射不敏感)和CNE-2(放射敏感)系鼻咽癌移植瘤模型,并分组进行分割照射,对每组进行MRI-DKI扫描。检测移植瘤乏氧相关因子(HIF-1α和VEGF)的表达,并测量DKI参数(D和K)值,观察并比较不同放射敏感性移植瘤放疗过程中各参数的变化以及DKI参数与乏氧相关因子之间的相关性。结果CNE-1和CNE-2移植瘤在放疗期间D值增高[(1.42±0.21)~(1.70±0.02)×10^(-3)mm^(2)/s、(1.32±0.07)~(2.24±0.10)×10^(-3)mm^(2)/s;F=4.647、162.360,均P<0.01],而K值降低(0.86±0.06~0.65±0.05、0.95±0.04~0.29±0.02;F=18.903、261.487,均P<0.001)。CNE-1系移植瘤中HIF-1α和VEGF的表达均增加(HIF-1α:1.0±0~5.79±2.35、60.52±4.55~166.20±24.55,VEGF:1.0±0~4.17±1.88、77.47±6.03~165.51±39.37;F=173.432、265.687、198.682、152.419,均P<0.001),CNE-2系移植瘤HIF-1α和VEGF表达先增加,中间下降,最后再增加。CNE-1中D值的变化与HIF-1α、VEGF呈正相关(r=0.598~0.618,均P<0.001);而CNE-2与HIF-1α和VEGF的相关性较弱(r=0.193~0.582);K值的变化与HIF-1α和VEGF水平呈负相关(r=-0.837~-0.477,均P<0.01)。CNE-2中仅K值与HIF-1α和VEGF相关。结论DKI具有早期检测肿瘤乏氧和放疗疗效的潜在能力,其中K值比D值的应用价值更大。Purpose To explore the application value of diffusion kurtosis imaging(DKI)parameters in evaluating tumor hypoxia and the effects of radiotherapy on nasopharyngeal carcinoma xenografts in nude mice.Materials and Methods Models of nude mice with CNE-1(radiation-insensitive)and CNE-2(radiation-sensitive)nasopharyngeal carcinoma xenograft were constructed and divided into different groups for irradiation.MRI-DKI scans were then performed on each group.The expressions of hypoxia-related biomarkers[(hypoxia-inducible factor-1α(HIF-1α)]and vascular endothelial growth factor(VEGF)]in xenografts were detected and the values of DKI parameters(D and K)were measured for statistical analysis.The DKI parameters,hypoxia-related biomarkers of CNE-1 and CNE-2 xenograft during radiotherapy were compared.The correlation between DKI parameters and hypoxia-related biomarkers was analyzed.Results The D value of CNE-1 and CNE-2 xenografts were increased during radiotherapy[(1.42±0.21)-(1.70±0.02)×10^(-3)mm^(2)/s,(1.32±0.07)-(2.24±0.10)×10^(-3)mm^(2)/s;F=4.647,162.360,P<0.01],while the K value were decreased(0.86±0.06-0.65±0.05,0.95±0.04-0.29±0.02;F=18.903,261.487,P<0.001).In addition,the expressions of HIF-1αand VEGF were increased in CNE-1 xenograft tumors(HIF-1α:1.0±0-5.79±2.35,60.52±4.55-166.20±24.55,VEGF:1.0±0-4.17±1.88,77.47±6.03-165.51±39.37;F=173.432,265.687,198.682,152.419,all P<0.001).The expressions of HIF-1αand VEGF in CNE-2 xenografts were increased at first and then decreased,finally increased.The change of D value in CNE-1 was positively correlated with HIF-1αand VEGF(r=0.598-0.618,all P<0.001),while the correlation between D value and HIF-1αor VEGF was weak in CNE-2(r=0.193-0.582);the change of K value was negatively correlated with HIF-1αand VEGF levels(r=-0.837--0.477,all P<0.01).Only K value in CNE-2 was related to HIF-1αand VEGF.Conclusion DKI has the potential ability in early detecting tumor hypoxia and radiotherapy efficacy.In addition,parameter K is more valuable than parameter D.
