摘 要:目的观察针刺透穴法对糖尿病周围神经病变(DPN)大鼠坐骨神经细胞凋亡相关蛋白表达的影响,探讨该疗法对DNP大鼠坐骨神经损伤修复作用及其可能作用机制。方法取6只健康雄性SD大鼠作为空白组,另取12只健康雄性SD大鼠采用高脂高糖饲养6周后单次腹腔注射链脲佐菌素(STZ)的方法建立DPN模型。将造模成功大鼠随机分为模型组和电针组,每组6只。电针组大鼠予阴陵泉透刺阳陵泉、内关透刺外关,隔日1次,每次20 min,连续干预7次,空白组和模型组大鼠仅捆绑不电针。干预结束后,检测各组大鼠血糖水平,HE染色法观察坐骨神经病理形态,Western blot法和免疫组化染色法检测坐骨神经中Bcl-2、Bax表达情况。结果与空白组比较,模型组大鼠血糖水平和坐骨神经中Bax蛋白相对表达量及平均光密度值均明显升高(P均<0.05),坐骨神经中Bcl-2蛋白相对表达量及平均光密度值均明显降低(P均<0.05);与模型组比较,电针组血糖水平变化不明显(P>0.05),坐骨神经中Bax蛋白相对表达量及平均光密度值均明显降低(P均<0.05),坐骨神经中Bcl-2蛋白相对表达量及平均光密度值均明显升高(P均<0.05)。HE染色显示电针组大鼠坐骨神经髓鞘结构损伤脱失的程度较模型组轻。结论针刺透穴法可上调坐骨神经Bcl-2的表达和下调Bax的表达,抑制DPN大鼠坐骨神经细胞过度凋亡,这可能是针刺透穴法修复DNP大鼠坐骨神经损伤、延缓DNP进展的机制之一。Objective It is to observe the effect of point-through-point acupuncture on sciatic nerve cell apoptosis related proteins in diabetic peripheral neuropathy(DPN)rats,and to explore its effect on sciatic nerve injury repair in DNP rats and its possible mechanism.Methods Six healthy male SD rats were selected as the blank group,and another 12 healthy male SD rats were selected to establish the DPN models by single intraperitoneal injection of streptozotocin(STZ)after 6 weeks of high-fat and high-sugar feeding.The successfully modeled rats were randomly divided into a model group and an electroacupuncture group,with 6 rats in each group.The rats in the electric acupuncture group were treated with penetrating acupuncture for SP9 to GB34 and PC6 to SJ5,once every other day,20 minutes each time,continuously treated for 7 times.The rats in the blank and model groups were only bundled without electroacupuncture.After the intervention,the level of blood glucose of the rats in each group was measured,the pathological morphology of sciatic nerve was observed by HE staining,and the expression of Bcl-2 and Bax in sciatic nerve was detected by Western blot method and immunohistochemical staining.Results Compared with the blank group,the level of blood glucose and the relative expression of Bax protein and the mean optical density value in the sciatic nerve were significantly increased in the model group(all P<0.05),and the relative expression of Bcl-2 protein and the mean optical density value in the sciatic nerve were significantly decreased in the model group(P<0.05);compared with the model group,the change of blood glucose level was not significant in the electroacupuncture group(P>0.05),and the relative expression of Bax protein and the mean optical density value in the sciatic nerve were significantly decreased(P<0.05),the relative expression of Bcl-2 protein and the mean optical density in the sciatic nerve were significantly increased(P<0.05).HE staining showed that the damage of myelin structure in the sciatic nerve of
