双眼原发性眼附属器套细胞淋巴瘤眼内浸润1例

摘　　要：患者男性, 因左眼进行性视物不清伴视物变形1个月就诊眼科。影像学表现为典型眼内淋巴瘤浸润, 头颅MRI和正电子发射CT未见全身其他部位肿物。追诉病史, 患者院外就诊时, 首先表现为泪腺肿大、眼球突出和眼外肌水肿等眼外的体征, 眼底无异常改变。结合病史, 确诊为双眼原发性眼附属器套细胞淋巴瘤眼内浸润。患者全身用利妥昔单抗联合泽布替尼治疗9个月后双眼视力明显提高, 眼内外表现消失。A male patient presented to department of ophthalmology for progressive left eye blurred vision with visual deformation for 1 month.Imaging findings were typical intraocular lymphoma infiltration,and head MRI and positron emission tomography-CT showed no mass in other parts of the body.After review of the medical history,the patient first presented with extrocular signs such as lacrimal gland enlargement,eyeball protrusion and extraocular myoedema,without any abnormal changes in the fundus.Combined with the history,the patient was diagnosed as a binocular primary accessory mantle cell lymphoma with intraocular infiltration.After 9 months of systemic treatment with rituximab combined with zbrutinib,the patient′s binocular visual acuity was significantly improved and the intraocular and intraocular manifestations disappeared.
