深部热疗配合胸腹腔内注射顺铂及甘露聚糖肽在恶性胸腹水中的疗效及对生活质量的影响

摘　　要：目的:探究深部热疗配合胸腹腔内注射顺铂及甘露聚糖肽在恶性胸腹水中的疗效及对生活质量的影响。方法:选择广州市中西医结合医院2020年9月-2022年1月收治的100例恶性胸腹水患者,按照随机数字表法将其分为对照组和观察组,各50例。对照组进行胸腹腔内注射顺铂及甘露聚糖肽治疗,观察组则进行深部热疗配合胸腹腔内注射顺铂及甘露聚糖肽治疗。比较两组的治疗总有效率、毒副反应发生率及治疗前后的生活质量[卡氏(KPS)评分及世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评分]。结果:观察组的总有效率显著高于对照组,毒副反应发生率显著低于对照组,差异有统计学意义(P<0.05)。治疗前,两组的KPS评分及WHOQOL-BREF评分比较,差异无统计学意义(P>0.05);治疗后2、4周,观察组的KPS评分及WHOQOL-BREF评分显著优于对照组,差异有统计学意义(P<0.05)。结论:深部热疗配合胸腹腔内注射顺铂及甘露聚糖肽在恶性胸腹水中的疗效较好,有助于减轻相关毒副反应及提升生活质量,因此在恶性胸腹水患者中的应用价值相对较高。Objective:To investigate the effect of deep hyperthermia combined with intrathoracic and intraperitoneal injection of Cisplatin and Mannan Peptide in patients with malignant hydrothorax and ascites and influence for life quality.Method:A total of 100patients with malignant hydrothorax and ascites admitted to Guangzhou Integrated Traditional Chinese and Western Medicine Hospital from September 2020 to January 2022 were selected,they were divided into control group and observation group by the random number table method,50 cases in each group.The control group was treated with intrathoracic and intraperitoneal injection of Cisplatin and Mannan Peptide,the observation group was treated with deep hyperthermia combined with intrathoracic and intraperitoneal injection of Cisplatin and Mannan Peptide.Then the total effective rates,toxic and side effects rates,life quality[Karnofsky(KPS)score and WHO quality of life brief scale(WHOQOL-BREF)]score before and after the treatment of two groups were compared.Result:The total effective rate in observation group was significantly higher than that in control group,and the toxic and side effects rates were significantly lower than those in control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in KPS score and WHOQOL-BREF score between the two groups(P>0.05).At 2 and 4 weeks after treatment,KPS score and WHOQOLBREF score in the observation group were significantly better than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Deep hyperthermia combined with intrathoracic and intraperitoneal injection of Cisplatin and Mannan Peptide has a good effect in malignant hydrothorax and ascites,which is helpful to reduce the related toxic side effects and improve the quality of life.Therefore,it has a relatively high application value in patients with malignant hydrothorax and ascites.
