摘 要:目的 探讨基于认知平衡理论的院外健康教育对健康体检幽门螺旋杆菌(Helicobacterpylori,Hp)阳性患者遵医行为及Hp转阴率的影响。方法 2021年9月—2022年8月,选取在徐州市中心医院体检中心检出的90例Hp阳性患者,采用随机数字表法分为常规组(n=45,实施常规健康护理)与研究组(n=45,实施基于认知平衡理论的院外健康教育)。评估2组患者干预前后的疾病不确定感、健康素养,比较2组患者的遵医行为及Hp转阴率。结果干预后研究组患者的疾病不确定感量表(MUIS-A)总分为(35.16±3.53)分,低于常规组的(48.59±4.37)分(t=11.085,P<0.001),健康素养总分为(51.95±6.93),高于常规组的(42.49±5.32)分(t=16.037,P<0.001),遵医行为率为100.00%、Hp转阴率为95.56%,高于常规组的80.00%、71.11%(χ^(2)_(遵医行为)=9.680、χ^(2)_(Hp转阴率)=10.000,P=0.002、0.002)。结论基于认知平衡理论的院外健康教育可提高健康体检Hp阳性患者的疾病认知,降低其疾病不确定感,提高患者的健康素养,改善患者的遵医行为,从而提高患者的Hp转阴率。ObjectiveTo explore the effect of out-of-hospital health education based on cognitive balance theory on medical compliance behavior and Helicobacter pylori(Hp) conversion rate of Hp-positive patients during health examination.MethodsFrom September 2021 to August 2022, 90 patients with Hp-positive detected in the Physical Examination Center of Xuzhou Central Hospital were divided into a routine group(n = 45, implementing routine health care) and a study group(n = 45, implementing out-of-hospital health education based on cognitive balance theory) by the random number table method.The disease uncertainty and health literacy of the patients before and after the intervention were evaluated, and the medical compliance behavior and Hp conversion rate of the two groups were compared.ResultsAfter the intervention, the total score of MUIS-A in the study group was(35.16 ± 3.53), lower than that in the conventional group:(48.59 ± 4.37)(t = 11.085,P < 0.001), and the total score of health literacy in the study group was(51.95 ± 6.93), higher than that in the conventional group:(42.49 ± 5.32)(t = 16.037, P < 0.001). The compliance rate in the study group was 100.00%, and the Hp-negative rate was 95.56%, higher than those in the routine group:(80.00%, 71.11%)(χ^(2)_(Compliance behavior)= 9.680, χ^(2)_(Hp-negative conversion rate) =10.000, P = 0.002, 0.002).ConclusionsThe out-of-hospital health education based on cognitive balance theory can improve the disease cognition of patients with Hp-positive in health examination, reduce their sense of disease uncertainty,improve patients’ health literacy and medical compliance behavior, and increase the rate of Hp turning negative.
