摘　　要：目的:分析早期急性脑梗死患者采用动静脉联合溶栓与机械取栓治疗的价值。方法:选择重庆市梁平区人民医院2019年2月—2022年2月收治的80例早期急性脑梗死患者作为研究对象,并按随机数表法分两组,各40例。对照组给予动静脉联合溶栓治疗,观察组给予机械取栓治疗,对比两组的临床疗效、神经功能缺损程度、预后效果、血清指标。结果:观察组治疗总有效率为95.00%,高于对照组的80.00%(χ^(2)=4.114,P<0.05)。治疗后3 d、2周、4周观察组美国国立卫生研究院卒中量表(NIHSS)评分低于对照组(P<0.05)。观察组24 h颅内出血率低于对照组,血管残余狭窄≤30%率高于对照组(P<0.05);观察组的神经元特异性烯醇化酶(NSE)、S100钙结合蛋白B(S100B)水平低于对照组,脑源性神经营养因子(BDNF)、血管内皮生长因子(VEGF)水平高于对照组(P<0.05)。结论:选择机械取栓治疗早期急性脑梗死可改善其预后效果,并促进损伤神经功能恢复,值得借鉴。Objective:To analyze and compare the value of early acute cerebral infarction patients with arteriovenous combined thrombolysis and mechanical thrombectomy.Method:A total of 80 patients with early acute cerebral infarction admitted to Liangping District People’s Hospital of Chongqing from February 2019 to February 2022 were selected as the research objects,and they were divided into two groups according to the random number table method,with 40 cases in each group.The control group was treated with arteriovenous combined thrombolysis,and the observation group was treated with mechanical thrombectomy.The clinical efficacy,degree of neurological deficit,prognosis and serum indicators of the two groups were compared.Result:The total effective rate of treatment in the observation group was 95.00%,which was higher than 80.00%in the control group(χ^(2)=4.114,P<0.05).At 3 days,2 weeks and 4weeks after treatment,the score of National Institute of Health stroke scale(NIHSS)in the observation group was lower than that in the control group(P<0.05).The 24 h intracranial hemorrhage rate in the observation group was lower than that in the control group,and the rate of residual vascular stenosis≤30%was higher than that in the control group(P<0.05).The levels of neuron specific enolase(NSE),S100 calbindin B(S100B)in observation group were lower than those in control group,the levels of brain derived neurotrophic factor(BDNF)and vascular endothelial growth factor(VEGF)were higher than those in the control group(P<0.05).Conclusion:Mechanical thrombectomy can improve the prognosis of early acute cerebral infarction and promote the recovery of injured nerve function,which is worthy of reference.
