五行音乐疗法联合碳酸锂片在双相情感障碍恢复期患者中的应用效果

摘　　要：目的观察五行音乐疗法联合碳酸锂片在双相情感障碍(BDP)恢复期患者中的应用效果。方法抽取2020年1月至2021年12月在山东省精神卫生中心住院治疗的80例BDP恢复期患者,按随机数字表法分为研究组和对照组,每组40例。对照组给予碳酸锂片治疗,研究组在对照组的基础上进行五行音乐疗法。比较两组疗效,治疗前后贝克-拉范森躁狂量表(BRMS)、汉密尔顿抑郁量表(HAMD)、临床疗效总评量表(CGI)、中医五态情志问卷-情绪词汇评分、中医五态情志问卷-躯体化症状评分和症状自评量表(SCL-90)评分,治疗后复发率。结果治疗4周后,研究组治疗总有效率(87.50%,35/40)与对照组(77.50%,31/50)比较差异未见统计学意义(P>0.05),但两组BRMS评分、HAMD评分、CGI评分、SCL-90评分、中医五态情志问卷-情绪词汇评分、中医五态情志问卷-躯体化症状评分均较治疗前显著下降(P均<0.05),且研究组低于对照组(P<0.05)。治疗后,研究组总复发率(5.00%,2/40)低于对照组(20.00%,8/40),P<0.05。结论五行音乐疗法结合碳酸锂片可有效改善BDP恢复期患者的抑郁、狂躁症状及病情的严重程度,缓解患者情志失调,降低治疗后复发率。Objective To observe the curative effect of five-element music therapy combined with lithium salt on patients with bipolar affective disorder(BDP)in recovery period.Methods Eighty patients with BDP in recovery period who were treated in Shandong Mental Health Center from January 2020 to December 2021 were selected.According to random number table method,patients were divided into study group and control group,with 40 cases in each group.Patients in the control group were treated with lithium salt.On this basis,patients in the study group were treated by five-element music therapy.The curative effects of the two groups were compared.The preoperative and postoperative Bech-Rafaelsen mania rating scale(BRMS)scores,Hamilton depression scale(HAMD)scores,clinical global impression(CGI)scores,traditional Chinese medical(TCM)five-state emotions questionnaire-emotional vocabulary scores,TCM five-state emotions questionnaire-somatization symptom scores,symptom checklist-90(SCL-90)scores,and postoperative recurrence rate of the two groups were compared.Results There was no significant difference in curative effect rate between the study group(87.50%,35/40)and the control group(77.50%,31/50)after 4 weeks of treatment(P>0.05).The scores of BRMS,HAMD,CGI,SCL-90,TCM five-state emotions questionnaire-emotional vocabulary and somatization symptoms were decreased in the two groups after 4 weeks of treatment,compared with the scores before treatment(all P<0.05),and the study group had lower scores than the control group(P<0.05).After treatment,the total recurrence rate in the study group(5.00%,2/40)was lower than that in the control group(20.00%,8/40),P<0.05.Conclusions Five-element music therapy combined with lithium salt can effectively improve the depression,manic state and severity of BDP patients in recovery period,and reduce the recurrence rate after treatment.
