单孔胸腔镜肺癌根治术对肺癌患者免疫功能的影响

摘　　要：目的探析单孔胸腔镜(VATS)肺癌根治术对肺癌患者免疫功能的影响。方法将80例早期肺癌患者随机分为观察组40例和对照组40例。对照组患者实施三孔VATS肺癌根治术,观察组患者实施单孔VATS肺癌根治术。比较两组患者围手术期相关指标,术前及术后1、3、7天炎症因子[C反应蛋白(CRP)、降钙素原(PCT)]、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平。结果观察组患者术中出血量少于对照组,术后VAS评分、肺炎发生率均低于对照组,差异均有统计学意义(P﹤0.05)。术后1、3天,两组患者CRP、PCT均高于本组术前,但观察组患者CRP、PCT均低于对照组,差异均有统计学意义(P﹤0.05)。术后1、3天,两组患者CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)均高于本组术前,差异均有统计学意义(P﹤0.05);术后1、3、7天,观察组患者CD3^(+)、CD4^(+)水平均高于对照组,术后1天,观察组患者CD4^(+)/CD8^(+)高于对照组,差异均有统计学意义(P﹤0.05)。结论相较于三孔VATS肺癌根治术,单孔VATS肺癌根治术临床疗效更显著,更有利于减少手术创伤及感染风险,缓解术后疼痛,改善感染性炎症反应及免疫功能抑制,值得临床推广。Objective To investigate the effect of single-port video assisted thoracic surgery(VATS)radical resection of lung cancer on the immune function of patients with lung cancer.Method A total of 80 patients with early lung cancer were randomly divided into observation group of 40 cases and control group of 40 cases.Patients in the control group received triple-port VATS radical resection of lung cancer,and patients in the observation group received single-port VATS radical resection of lung cancer.Perioperative indicators,inflammatory factors[C-reactive protein(CRP),procalcitonin(PCT)],T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))levels preoperative and postoperative 1,3 and 7 days were compared between the two groups.Result The intraoperative blood loss volume in the observation group was less than that in the control group,and the postoperative VAS score and the incidence of pneumonia in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).On the first and third days after the operation,the CRP and PCT in the two groups were higher than those before the operation,but both indexes in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).On the first and third days after the operation,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the two groups were higher than those before the operation,and the differences were statistically significant(P<0.05).On the first,third,and seventh days after the operation,the levels of CD3^(+)and CD4^(+)in the observation group were higher than those in the control group;one day after the operation,the CD4^(+)/CD8^(+)in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with triple-port VATS radical resection of lung cancer,single-port VATS radical resection of lung cancer has more significant clinical efficacy and is more conducive to red
