摘 要:目的 了解衡阳市食源性疾病沙门菌血清型分布、脉冲场凝胶电泳(pulsed-field gel electrophoresis, PFGE)分型及耐药特征。方法 对2019年—2021年食源性疾病患者的粪便/肛拭子进行沙门菌分离鉴定及血清分型,微量肉汤稀释法进行药敏实验,PFGE分型,结果用SPSS 18.0软件进行统计分析,BioNumerics软件聚类分析。结果 沙门菌总检出率为16.3%,不同年份检出率差异无统计学意义(χ^(2)=0.169,P=0.919);3岁以下婴幼儿检出率最高,为65.1%;男性检出率(61.1%)高于女性(38.9%)。血清型以鼠伤寒沙门菌(53.0%)占优势。所有沙门菌对亚胺培南敏感,对四环素耐药最高;耐3种及以上抗生素的菌株占65.1%;3年间耐药情况无明显变化。79株鼠伤寒沙门菌共产生64种PFGE带型,相似度为71.3%~100%,有12对完全相同的PFGE图谱。结论 衡阳市2019年—2021年食源性沙门菌检出率较高,3岁以下婴幼儿检出率最高,耐药严重,PFGE带型呈多态性分布。Objective This study aims to understand the distribution of Salmonella serotypes, pulsed-field gel electrophoresis(PFGE) typing and drug resistance characteristics of foodborne diseases in Hengyang City. Methods From 2019 to 2021, Salmonella isolation, identification and serotyping were carried out from stool/anal swabs of patients with foodborne diseases, drug susceptibility test was carried out by micro-broth dilution method, and PFGE typing was carried out. The results were statistically analyzed with SPSS 18.0 software, and BioNumerics software was adopted for cluster analysis. Results The total detection rate of Salmonella was 16.3%, and there was no statistical significance on the difference in the detection rate in different years(χ^(2)=0.169, P=0.919). The detection rate of infants under 3 years old was the highest(65.1%);the detection rate in males(61.1%) was higher than that in females(38.9%). The serotype was dominated by Salmonella typhimurium(53.0%). All Salmonella were sensitive to imipenem, with the highest resistance to tetracycline;65.1% of the strains were resistant to three or more antibiotics;there was no significant change in drug resistance in the three years. 79 strains of Salmonella typhimurium produced a total of 64 PFGE band types, the similarity was within 71.3%-100%, and 12 pairs of identical PFGE patterns were observed. Conclusion The detection rate of foodborne Salmonella in Hengyang City from 2019 to 2021 was relatively high, and the detection rate in infants and young children under 3 years old was the highest. Drug resistance is severe, and the PFGE band type is polymorphic.
