摘  要:目的:探究在重症肌无力危象治疗中,实施无创机械通气的临床效果与价值。方法:选取2019年1月~2020年7月本院收治的42例重症肌无力危象患者为此次临床观察对象;在患者入院后均予以无创机械通气治疗,观察患者在治疗前后(治疗后48h)的呼吸、心率指标以及气血分析情况(主要观察患者血浆pH值以及QMG、PaO_(2)、PaCO_(2)等指标水平)。结果:通过无创机械通气治疗后,本组42例患者的心率及呼吸频率分别为(82.77±12.75)次/min、(21.56±4.78)次/min,对比治疗前的(116.25±12.43)次/min、(24.45±7.10)次/min均有显著改善,P<0.05;观察其血气分析结果可见:其治疗前后的血浆pH值并无明显变化[(7.30±0.05)vs(7.31±0.04)],P>0.05;而治疗后的QMG、PaO_(2)、PaCO_(2)等指标水平分别为(10.05±2.77)分、(83.34±15.82)mmHg、(38.56±7.16)mmHg,对比治疗前的(17.93±2.06)分、(58.66±13.41)mmHg、(53.74±7.96)mmHg存在明显变化,P<0.05。结论:在重症肌无力危象治疗中,实施无创机械通气可显著改善患者的心率、呼吸频率,促进患者病情的改善,使其动脉气血情况趋于稳定,降低患者的痛苦,治疗价值显著。Objective: To explore the clinical effect and value of non-invasive mechanical ventilation in the treatment of myasthenia gravis crisis. Methods: 42 patients with myasthenia gravis crisis admitted to our hospital from January 2019 to July 2020 were selected for this clinical observation;All patients were treated with non-invasive mechanical ventilation after admission, and the respiratory, heart rate and blood analysis of the patients before and after treatment(48h after treatment) were observed(mainly the plasma p H value and the levels of QMG, PaO_(2), PaCO_(2)and other indicators were observed). Results: After noninvasive mechanical ventilation, the heart rate and respiratory rate of 42 patients were(82.77±12.75) times/min,(21.56±4.78) times/min, compared with(116.25±12.43) times/min,(24.45±7.10) times/min(P<0.05);The results of blood gas analysis showed that there was no significant change in plasma p H value before and after treatment(7.30±0.05)vs(7.31±0.04)(P>0.05);After treatment, the levels of QMG, PaO_(2)and PaCO_(2)were(10.05±2.77)points,(83.34±15.82)mm Hg,(38.56±7.16)mm Hg, compared with(17.93±2.06)points,(58.66±13.41)mm Hg,(53.74±7.96) mm Hg(P<0.05). Conclusion: In the treatment of myasthenia gravis crisis, the implementation of non-invasive mechanical ventilation can significantly improve the patient’s heart rate and respiratory rate, promote the improvement of the patient’s condition, stabilize the arterial blood and reduce the patient’s pain, and have significant therapeutic value.
