血清甘胆酸、甲胎蛋白联合乙型肝炎病毒前C区/BCP区突变检测在肝细胞肝癌中的临床价值

摘　　要：目的:探讨血清甘胆酸、甲胎蛋白(α-fetoprotein, AFP)和乙型肝炎病毒前C区/BCP区突变的关系及对肝细胞肝癌(Hepatocellular carcinoma, HCC)诊断的意义。方法:收集2020年12月—2022年2月在我院诊疗的HCC患者200例,分为乙肝肝癌组(100例)和非乙肝肝癌组(100例)。采用免疫比浊法检测血清甘胆酸、双抗体夹心法检测血清AFP、PCR-反向点杂交法检测HBV前C区/BCP区突变,通过SPSS 22.0软件进行联合分析。结果:乙肝肝癌组的血清AFP水平显著高于非乙肝肝癌组的血清AFP(P<0.05);在乙肝肝癌组中,HBV基因突变组血清AFP高于HBV基因未突变组(P<0.05),其中BCP区突变组AFP水平高于BCP区未突变组(P<0.05)。结论:血清AFP水平与HCC类型和HBV突变类型有潜在相关性,对HCC的发病机制了解和诊断有重大参考价值。Objective : To explore the relationship between cholyglycine, AFP and mutations in the pre-C region/BCP region of hepatitis B virus, and its significance in the diagnosis of hepatocellular carcinoma(HCC). Methods : From December 2020 to February 2022, a total of 200 cases with HCC in our hospital were divided into hepatitis B group(100cases) and non-hepatitis B group(100 cases). Immunoturbidimetric method was used to detect cholyglycine, double anti-body sandwich method to detect AFP, PCR-reverse dot blot to detect the mutation of HBV pre-C region/BCP region, and SPSS 22. 0 software was used for conjoint analysis. Results : The AFP level in the hepatitis B group was significantly higher than that in the non-hepatitis B group(P<0. 05). Among the hepatitis B group, the AFP of the HBV mutation group was higher than that of the non-mutated group(P<0. 05), and the level of AFP in the mutant group was higher than that in the non-mutated group in the former C region(P<0. 05). Conclusion : AFP has potential correlations with HCC types and HBV mutation types, and has great reference value for understanding and diagnosing the disease.
