摘  要:目的探讨高分辨率磁共振血管壁成像(HRMR-VWI)技术联合磁共振血管成像(MRA)对颅内动脉瘤(IA)瘤体破裂的诊断价值。方法回顾性分析2019年5月至2021年5月南阳南石医院收治的51例破裂IA患者的临床资料,并将其纳入破裂组;另抽取同期收治的51例未破裂IA患者纳入未破裂组。全部患者均接受HRMR-VWI及MRA检查,对比观察HRMR-VWI及MRA检查IA破裂与未破裂患者的影像学特征、参数,并分析HRMR-VWI技术联合MRA检查对IA破裂风险的评估价值。结果破裂组MRA检查中瘤体最大径大于未破裂组(P<0.05);破裂组HRMR-VWI检查中瘤壁强化等级高于未破裂组,伴有子囊占比高于未破裂组,瘤体最大径、体颈比、高颈比均高于未破裂组(P均<0.05),组间其他资料比较差异未见统计学意义(P>0.05)。绘制受试者工作特征曲线结果显示,MRA参数(瘤体最大径)、HRMR-VWI参数(瘤壁强化等级、子囊、瘤体最大径、体颈比、高颈比)评估IA破裂风险的曲线下面积>0.70,具有一定的评估价值,且当MRA瘤体最大径、HRMR-VWI瘤体最大径、体颈比、高颈比的最佳截断值分别取6.700 mm、6.750 mm、1.280、1.820时,可获得最佳评估价值。结论HRMR-VWI技术联合MRA可准确鉴别IA破裂瘤体,并对IA破裂风险具有较高的评估价值。Objective To observe the value of high-resolution magnetic resonance vessel wall imaging(HRMR-VWI)combined with magnetic resonance angiography(MRA)in the diagnosis of ruptured intracranial aneurysms(IA).Methods The clinical data of 51 patients with ruptured IA admitted to Nanshi Hospital of Nanyang from May 2019 to May 2021 were retrospectively analyzed,and patients were enrolled into the ruptured group;in addition,51 patients with unruptured IA admitted in the same period were enrolled into the unruptured group.All patients underwent HRMR-VWI and MRA.The imaging characteristics and parameters of HRMR-VWI and MRA of patients with ruptured and unruptured IA were compared,and the value of HRMR-VWI combined with MRA in evaluation on the risk of ruptured IA was mainly analyzed.Results The maximum diameter of tumor in MRA of the ruptured group was bigger than that of the unruptured group(P<0.05);in HRMR-VWI,the enhancement grade of the tumor wall in the ruptured group was higher than that in the unruptured group,and the proportion of subcapsules in the rupture group was higher than that in the unruptured group,the maximum diameter of tumor,body neck ratio and height neck ratio were higher than those in the unruptured group(all P<0.05);there was no significant difference in other data between the two groups(P>0.05).The results of the receiver operating characteristic curve showed that the area under the curve of MRA parameters(maximum diameter of tumor)and HRMR-VWI parameters(enhancement grade of tumor wall,subcapsule,maximum diameter of tumor,ratio of body to neck,ratio height of neck)in evaluating the risk of ruptured IA were>0.70,which had certain evaluation value;when the cut-off values of MRA tumor maximum diameter,HRMR-VWI tumor maximum diameter,body neck ratio and height neck ratio were 6.700 mm,6.750 mm,1.280 and 1.820,respectively,the best evaluation value could be obtained.Conclusions HRMR-VWI combined with MRA can accurately identify IA ruptured tumors,and has high value in evaluating the risk of ruptured IA
