帕金森病轻度认知功能障碍相关危险因素探讨

摘　　要：目的探讨帕金森病(PD)轻度认知功能障碍(PD-MCI)的相关危险因素,进而优化PD管理。方法收集河北医科大学第二医院神经内科PD患者57例为实验组(PD组),根据认知功能分为PD-MCI组、PD认知正常(PD-NC)组,健康人群92例为对照组(HC组)。收集基本资料,采集静脉血测定血脂(TC、TG、HDL、LDL)、胆红素(TBIL、DBIL、IBIL)、尿酸(UA)、同型半胱氨酸(Hcy),PD组评估统一PD评定量表第三部分(UPDRS-Ⅲ)、Hoehn-Yahr分期(H-Y分期)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)、简易精神状态量表(MMSE)、蒙特利尔认知量表(MoCA),并进行相关回归分析,探讨PD-MCI危险因素。结果PD-MCI组UPDRS-Ⅲ评分、HAMA评分、HAMD评分显著高于PD-NC组(P<0.05);PD-MCI组较PD-NC组血生化指标无显著差别。PD组血脂、UA较HC组显著降低,DBIL、Hcy显著高于HC组(P<0.05)。多因素Logistic回归分析中UPDRS-Ⅲ评分是PD-MCI的独立危险因素(P<0.05)。结论UPDRS-Ⅲ评分是PD-MCI的独立危险因素。PD患者均存在血脂等生化指标异常。Objective To explore the related risk factors associated with mild cognitive impairment in Parkinson’s disease(PD-MCI)and to optimize the management of PD.Methods 57 PD patients in the department of neurology,the second hospital of Hebei medical university were collected as experimental group(PD group),divided into PD-MCI group and PD cognitive normal group(PD-NC)according to cognitive function,and 92 healthy people as control group(HC group).Basic data were collected and some blood biochemical indexes were detected,including blood lipids(TC,TG,HDL,LDL),bilirubin(TBIL,DBIL,IBIL),uric acid(UA),homocysteine(Hcy).Unified Parkinson’s disease rating scale-Ⅲ(UPDRS-Ⅲ),Hoehn-Yahr stage(H-Y stage),Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD),mini-mental state scale(MMSE)and Montreal cognitive scale(MoCA)were evaluated in PD group.Correlation and regression analyses were performed to investigate the risk factors for PDMCI.Results UPDRS-Ⅲscore,HAMA score and HAMD score in PD-MCI group were significantly higher than PD-NC group(P<0.05),but there was no statistical significance in above blood biochemical indexes between them.Compared with HC group,PD group had significantly lower serum lipids and UA,and higher DBIL and Hcy(P<0.05).Multivariate Logistic regression analysis showed that UPDRS-Ⅲscore was an independent risk factor for PD-MCI(P<0.05).Conclusions UPDRS-Ⅲscore is an independent risk factor for PD-MCI(P<0.05).Abnormal blood biochemical indexes such as lipids exist in PD patients.
