摘 要:目的初步探讨多巴丝肼对帕金森病(PD)患者的嗓音功能障碍的治疗效果。方法选择22例PD患者(15例男性和7例女性)在口服多巴丝肼前和用药后1h进行频闪喉镜检查和嗓音声学功能的主客观指标综合评估,同时本文招募了年龄和性别相匹配的健康对照组来进行比较。结果PD患者用药前后声门闭合情况、黏膜波和声带振动幅度与健康对照者比较,差异均有统计学意义(P<0.05);口服多巴丝肼后1h,声带黏膜波明显减弱的比例减少,用药前后比较差异有统计学意义(P<0.05)。PD患者在用药前基频、基频微扰、振幅微扰、嗓音障碍严重指数(客观评估)、嗓音障碍指数(主观评估)和GRBAS和健康对照组比较(包括男性和女性),差异有统计学意义(^(均)P<0.05);基频、振幅微扰和嗓音障碍严重指数在口服美多巴1h后与用药前比较,差异有统计学意义(P<0.05);用药后1h基频微扰、振幅微扰(男性)、嗓音障碍严重指数、嗓音障碍指数和GRBAS(G)与健康对照组比较差异有统计学意义(P<0.05)。结论多巴丝肼可以调节喉内外肌张力使其僵硬减轻从而改善PD患者的嗓音功能。Objective To investigate the efficacy of levodopa and benserazide hydrochloride on voice dysfunction in patients with Parkinson’s disease(PD).Methods Twenty two patients with PD(15 male and 7 female)were selected for stroboscopic laryngoscopy and comprehensively subjective and objective evaluation of voice acoustic function,before and 1 hour after oral levodopa and benserazide hydrochloride.We also recruited healthy controls matched by age and sex for comparison.Results There were significant differences in glottic closure,mucosal wave and vocal cord vibration amplitude between patients with PD and healthy controls before and after oral medication(P<0.05);one hour after oral administration of levodopa and benserazide hydrochloride,the proportion of weaken significantly vocal cord mucosal wave decreased,and the difference was statistically significant before and after oral medication(P<0.05).There were significant differences in fundamental frequency,jitter,shimmer,dysphonia severity index(objective evaluation),voice handicap index(subjective evaluation)and GRBAS between PD patients and healthy controls(including male and femal)(P<0.05);there were significant differences in fundamental frequency,shimmer and dysphonia severity index between 1 hour after and before oral medication(P<0.05);there were significant differences in jitter,shimmer(male),dysphonia severity index,voice handicap index and GRBAS(G)between the treatment group and the healthy control one hour after oral medication(P<0.05).Conclusion Levodopa and benserazide hydrochloride can partially improve the voice function of patients with Parkinson’s disease,which may be the result of regulating the tension and stiffness of internal and external laryngeal muscles.
