摘　　要：目的 探讨血清同型半胱氨酸(Hcy)、单核细胞趋化蛋白-1 (MCP-1)表达与缺血性小卒中(MIS)患者复发脑卒中的关系。方法 选取2018年1月至2021年1月万宁市人民医院收治的253例MIS患者,根据治疗后1年内是否复发脑卒中分为复发组和未复发组。对比两组患者基础资料及血清Hcy、MCP-1水平,分析Hcy、MCP-1对MIS患者复发脑卒中的预测价值。结果 253例MIS患者1年内复发脑卒中37例,复发率14.62%。复发组年龄及血清Hcy、MCP-1水平高于未复发组(P<0.05)。Logistic分析显示,血清Hcy和MCP-1水平是MIS患者复发脑卒中的独立危险因素(P<0.05)。血清Hcy联合MCP-1预测MIS患者复发脑卒中的曲线下面积为0.897,预测灵敏度为89.2%,特异为75.5%。结论 血清Hcy和MCP-1的表达与MIS患者复发脑卒中密切相关,对早期预测再发脑卒中风险具有参考价值。Objective To investigate the relationship between serum homocysteine(Hcy) and monocyte chemotactic protein-1(MCP-1) expression and recurrent stroke in patients with minor ischemic stroke(MIS).Methods A total of 253 MIS patients admitted to Wanning People’s Hospital from January 2018 to January 2021 were selected and divided into recurrence group and non-recurrence group according to whether they had recurrent stroke within 1 year after treatment.The basic information and serum Hcy and MCP-1 levels of patients in the two groups were compared.The predictive value of Hey and MCP-1 for recurrent stroke in MIS patients was analyzed.Results Among 253 MIS patients,37 patients had recurrence of stroke within 1 year,the recurrence rate was 14.62%.The age and serum Hcy,MCP-1 levels in the recurrence group were higher than those in the non-recurrence group(P<0.05).Logistic analysis showed that,serum Hey and MCP-1 levels were independent risk factors for recurrent stroke in MIS patients(P<0.05).The area under the curve of serum Hcy combined with MCP-1 in predicting recurrent stroke in MIS patients was 0.897,with a predictive sensitivity of 89.2% and specificity of 75.5%.Conclusion The expression of serum Hey and MCP-1 is closely related to the recurrence of stroke in MIS patients,which has reference value for the early prediction of the recurrent stroke risk.
