摘　　要：目的探讨视神经脊髓炎谱系疾病(NMOSD)患者外周血辅助性17型T细胞(Th17)细胞/调节性T细胞(Treg)失衡与肢体伤残的关系及其临床意义。方法收集2019年1月至2020年12月在新疆维吾尔自治区人民医院确诊为NMOSD患者19例和对照组20例,根据扩展残疾状况量表EDSS对视神经脊髓炎患者进行评分,用流式细胞术检测外周血Th17、Treg细胞比例和外周血白介素17(IL-17)和白介素10(IL-10)细胞因子水平,比较两组患者Th17、Treg细胞比例及其细胞因子的改变,分析Th17、Treg细胞比例及其细胞因子与患者肢体残疾的相关性。结果NMOSD组与对照组之间对比,Th17细胞比例差异无统计学意义(P>0.05);但NMOSD组中Treg细胞比例较对照组低,Th17/Treg比值较对照组高,差异有统计学意义(P<0.05)。两组之间IL-17和IL-10水平之间差异均无统计学意义(P>0.05),但IL-17/IL-10比值差异有统计学意义(P>0.05)。NMOSD患者急性期EDSS评分与Treg比例、IL-10水平及IL-17/IL-10比例均有相关性(P<0.05)。结论NMOSD患者外周血Th17/Treg细胞失衡可能与Treg下降相关,Treg细胞比例、IL-10水平及IL-17/IL-10比例与患者肢体伤残相关。Objective To investigate the peripheral blood helper T cell type 17(Th17)cells/regulatory T cell(Treg)cells in patients with neuromyelitis optica spectrum disorder(NMOSD).The relationship between imbalance and limb disability and its clinical significance.Methods From January 2019 to December 2020,19 patients with NMOSD diagnosed in Xinjiang Uygur Autonomous Region People’s Hospital and 20 patients in the control group were collected,and the patients with neuromyelitis optica were scored according to the Expanded Disability Status Scale(EDSS),and flow cytometry was used to detect the proportion of Th17 and Treg cells in peripheral blood and the levels of interleukin-17(IL-17)and interleukin-10(IL-10)cytokines in peripheral blood.Changes in the proportion of Treg cells and their cytokines,and the correlation between the proportion of Th17 and Treg cells and their cytokines and limb disability in patients was analyzed.Results There was no significant difference in the proportion of Th17 cells between the NMOSD group and the control group(P>0.05);however,the proportion of Treg cells in the NMOSD group was lower than that in the control group,and the ratio of Th17/Treg was higher than that in the control group,which was statistically significant(P<0.05).There was no significant difference in the levels of IL-17 and IL-10 between the two groups(P>0.05),but the difference in the ratio of IL-17/IL-10 was statistically significant(P<0.05).The acute phase EDSS score of NMOSD patients was correlated with Treg ratio,IL-10 level and IL-17/IL-10 ratio(P<0.05).Conclusion The imbalance of Th17/Treg cells in peripheral blood of NMOSD patients may be related to the decrease of Treg cells,and the proportion of Treg cells,the level of IL-10 and the ratio of IL-17/IL-10 are related to the limb disability of patients.
