TCT与活检病理对早期宫颈癌及宫颈上皮内病变的诊断价值

摘　　要：目的 探究薄层液基细胞学检查(TCT)与活检病理两种诊断方式在早期宫颈癌及宫颈上皮内病变鉴别诊断中的应用价值。方法 选择行宫颈癌筛查110例患者为研究对象,所选患者均予以TCT与活检病理。比较两种诊断方式的诊断结果以及准确性、敏感度、特异度。结果 TCT检出早期宫颈癌假阳性6例,宫颈上皮内病变假阴性5例;活检病理检出早期宫颈癌假阳性1例,宫颈上皮内病变假阴性1例,两种检查方式检出结果比较,差异有统计学意义(P<0.05)。活检病理在早期宫颈癌及宫颈上皮内病变鉴别诊断准确性、敏感度、特异度高于TCT检查,差异有统计学意义(P<0.05)。结论 TCT与活检病理在早期宫颈癌及宫颈上皮内病变诊断中均具有一定诊断价值,活检病理可获得更高的准确性、敏感度、特异度,可为早期宫颈癌筛查提供客观、精准数据参考,值得临床推广与应用。Objective To explore the diagnostic value of thin layer liquid based cytology(TCT) and biopsy pathology in the differential diagnosis of early cervical cancer and cervical intraepithelial lesions. Methods A total of 110 cases of cervical cancer screening were selected as subjects. All the selected patients received TCT and biopsy pathology. The diagnostic results of the two diagnostic methods were compared, and the accuracy, sensitivity and specificity of the two methods were compared. Results A total of 6 cases of early cervical cancer were false positive and 5 cases of cervical intraepithelial lesions were false negative. Biopsy pathology detected false positive in 1 case of early cervical cancer, and false negative in 1 case of cervical intraepithelial lesions, and the difference was statistically significant(P<0.05). The accuracy, sensitivity and specificity of biopsy pathology in the differential diagnosis of early cervical cancer and cervical intraepithelial lesions were higher than that of TCT, and the difference was statistically significant(P<0.05). Conclusion Both TCT and biopsy pathology have certain diagnostic value in the diagnosis of early cervical cancer and cervical intraepithelial lesions. Biopsy pathology can obtain higher accuracy, sensitivity and specificity, and can provide objective and accurate data reference for the screening of early cervical cancer, worthy of clinical promotion and application.
