摘　　要：目的探讨皮肤黑色素瘤(CMM)组织中神经前体细胞表达发育调控样蛋白(Nedd4L)和环指型泛素连接酶(Cbl-b)蛋白的表达及对患者预后的评估价值。方法选取40例CMM患者和50例良性痣患者分别作为CMM组和对照组,收集CMM组患者的年龄、性别及临床特征(发生部位、是否发生转移、临床分期及肿瘤是否有溃疡等)信息;取2组患者手术切除组织存档蜡块制作切片,采用链霉菌抗生物素蛋白-过氧化物酶连结法(SP)法检测2组患者Nedd4L和Cbl-b的表达;随访CMM组患者的3年生存情况,应用Cox单因素分析模型分析评估影响预后的独立因素,采用受试者工作特征(ROC)分析Nedd4L、Cbl-b单一和联合检测对CMM组患者预后的预测价值。结果Nedd4L和Cbl-b蛋白在CMM组中表达水平高于对照组(P<0.05),患者年龄、肿瘤发生转移、肿瘤溃疡、临床分期、肿瘤厚度、Nedd4L及Cbl-b阳性表达是影响CMM预后的关键因素(P<0.05);联合检测Nedd4L和Cbl-b蛋白对CMM患者预后的预测能力高于单个指标检测(P<0.05)。结论CMM患者病变组织中Nedd4L和Cbl-b蛋白呈高表达,2项指标联合检测可提高预后评价的准确性及灵敏性。Objective To investigate the expression and clinical significance of neural precursor cell expressed developmentally down-regulated 4-like(Nedd4 L)and asitasb-lineage lymphoma-b(Cbl-b)in the tissues of patients with cutaneous malignant melanoma(CMM)and its evaluating value for their prognosis.Methods Forty CMM patients and 50 benign nevus patients were collected as CMM group and control group;the information of age,gender and clinical characteristics(location,distant metastasis,clinical stage,tumor ulcer,etc.)of CMM patients were collected.The archived wax blocks from the two groups were severed,and the expression of Nedd4 L and Cbl-b in both groups was detected by Streptomyces antibiotin protein-peroxidase binding method(SP).Survival condition of patients after 3 years in the CMM group was followed up.Cox univariate analysis model was used to analyze and evaluate the independent factors affecting the prognosis;the receiver operating characteristic(ROC)was used to analyze the predictive value of single and combined tests by Nedd4 L and Cbl-b for the prognosis of CMM group patients.Results The expression level of Nedd4 L and Cbl-b protein in CMM group was significantly higher than that of control group(P<0.05).Age,tumor ulcer,tumor metastasis,clinical stage,tumor thickness,positive expression of Nedd4 L and Cbl-b were the key factors affecting the prognosis of CMM(P<0.05).The predictive ability of combined detection of NEDD4 L and Cbl-b protein in the prognosis of CMM patients was higher than that of single index detection(P<0.05).Conclusion Nedd4 L and Cbl-b protein are highly expressed in CMM tissues,and their combination can improve the accuracy and sensitivity of prognosis evaluation in CMM patients.
