多功能保留颈清扫术联合中央淋巴结清扫术在分化型甲状腺癌患者中的应用效果

摘　　要：目的:探究多功能保留颈清扫术联合中央淋巴结清扫术(CLND)在分化型甲状腺癌(DTC)患者中的应用效果。方法:选取2018年9月-2021年9月九江市第一人民医院收治的112例DTC患者,根据随机数字表法将患者分为观察组(n=56)和对照组(n=56)。对照组予以CLND,观察组在对照组的基础上予以多功能保留颈清扫术。比较两组手术时间、术中出血量、总住院时间、咽部与声音情况、术后并发症发生情况。结果:与对照组相比,观察组术中出血量较少,总住院时间较短,手术时间较长(P<0.05)。术后,观察组咽部反流症状指数量表(RSI)、嗓音障碍指数量表(VHI)评分均低于对照组(P<0.05)。观察组的并发症发生率为5.36%,低于对照组的8.93%,差异无统计学意义(P>0.05)。结论:予以DTC患者多功能保留颈清扫术联合CLND,可有效降低术中出血量及总住院时间,改善患者咽部与声音状况,且安全。Objective:To explore the application effect of multifunctional neck preserving dissection combined with central lymph node dissection(CLND)in patients with differentiated thyroid cancer(DTC).Method:A total of 112 DTC patients admitted to Jiujiang NO.1 People's Hospital from September 2018 to September 2021 were selected,they were divided into observation group(n=56)and control group(n=56)according to random number table method.The control group was given CLND,and the observation group was given multifunctional neck preserving dissection on the basis of the control group.Operation time,intraoperative blood loss,total length of hospital stay,pharynx and sound,postoperative complications were compared between the two groups.Result:Compared with the control group,the observation group had less intraoperative blood loss,shorter total length of hospital stay and longer operation time(P<0.05).After surgery,the pharyngeal reflux symptom index(RSI)and voice handicap index(VHI)scores in the observation group were lower than those in the control group(P<0.05).The incidence of complications in the observation group was 5.36%,which was lower than 8.93%in the control group,the difference was not statistically significant(P>0.05).Conclusion:Multifunctional neck preserving dissection combined with CLND in patients with DTC can effectively reduce the amount of intraoperative blood less and the total length of hospital stay,and improve the pharyngeal and sound status of patients,which is safe.
