清胰汤对急性胰腺炎患者急性时相反应蛋白及胰腺血流、功能的影响观察

摘　　要：目的:探究清胰汤对急性胰腺炎患者急性时相反应蛋白及胰腺血流、功能的影响。方法:将2020年1月-2022年3月常德市第一中医医院收治的80例急性胰腺炎患者根据随机数字表法分为对照组和观察组,各40例。对照组进行常规急性胰腺炎治疗,观察组在常规治疗的基础上加用清胰汤。比较两组治疗总有效率、治疗前后血清急性时相反应蛋白[C反应蛋白(CRP)、触珠蛋白(HP)及铜蓝蛋白(CER)]、胰腺血流[肠系膜上动脉(SMA)及胰十二指肠下动脉(IPA)最大血流速]、胰腺功能[淀粉酶(AMS)、脂肪酶(LPS)及胰高血糖素(GG)]及胰头、胰体尾厚度。结果:观察组治疗总有效率显著高于对照组,差异有统计学意义(P<0.05);治疗前两组血清急性时相反应蛋白、胰腺血流及胰腺功能相关指标、胰头及胰体尾厚度比较,差异均无统计学意义(P>0.05),治疗3、7 d后,观察组血清急性时相反应蛋白、胰头及胰体尾厚度、胰腺功能相关指标均显著低于对照组,SMA及IPA最大血流速均显著高于对照组,差异均有统计学意义(P<0.05)。结论:清胰汤在急性胰腺炎患者中的应用效果较好,可显著减轻患者炎症反应,改善胰腺血流和功能,应用价值较高。Objective:To explore the effect of Qingyi Decoction on acute phase reactive protein,pancreatic blood flow and function in patients with acute pancreatitis.Method:A total of 80 patients with acute pancreatitis from January 2020 to March 2022 in Changde First Hospital of Traditional Chinese Medicine were randomly divided into a control group and an observation group according to the random number table method,40 cases in each group.The control group was treated with routine acute pancreatitis,and the observation group was treated with Qingyi Decoction on the basis of routine treatment.The total effective rate,serum acute phase reaction protein[C reactive protein(CRP),haptoglobin(HP)and ceruloplasmin(CER)],pancreatic blood flow[maximum blood flow velocity of superior mesenteric artery(SMA)and inferior pancreaticoduodenal artery(IPA)],pancreatic function[amylase(AMS),lipase(LPS)and glucagon(GG)],the thickness of pancreatic head and tail were compared between the two groups before and after treatment.Result:The total effective rate in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences between the two groups in serum acute phase reactive protein,pancreatic blood flow and pancreatic function related indexes,pancreatic head and pancreatic tail thickness(P>0.05).After treatment for 3,7 d,the serum acute phase reactive protein,pancreatic head and pancreatic tail thickness and pancreatic function related indexes in the observation group were significantly lower than those in the control group,and the maximum blood flow rates of SMA and IPA were significantly higher than those in the control group(P<0.05).Conclusion:Qingyi Decoction has a good application effect in patients with acute pancreatitis,which can significantly reduce the inflammatory reaction of patients,improve the pancreatic blood flow and function,and has high application value.
