摘  要:目的主要构建经食管超声心动图(TEE)监测心脏功能的动物模型,包括麻醉方法与主要切面获得;使用二维斑点追踪显像技术(2D-STI)和Qlab软件分析猪心脏左心室和右心室心肌应变力及同步化运动生理参数、与生理参数的可信区间;分析每搏量(SV)、左心室射血分数(LEVF)与左心室整体纵向应变力(GLS)的相关性。方法选择10只健康实验家猪,猪龄12~18个月,体质量45~60 kg。在全身麻醉下记录以下数据,血流动力学指标心率(HR)、平均动脉压(MAP)、SV、心排血量(CO)、LVEF、右心室射血分数(RVEF)。采用2D-STI行TEE。TEE监测指标包括在经食管中段四腔心、两腔心、三腔心切面分析得到的左心室纵向应变力(LS-ME4、LS-ME2、LS-ME3)及其平均值GLS,记录各节段收缩期纵向应变达峰时间并计算其标准差(GL Time SD);右心室纵向应变力(LS-RV)与收缩期达峰时间的标准差(RV Time SD);采用经胸超声心电图(TTE)在左心室短轴切面,分析得出心肌环向应变力(MCS)、左心室环向应变收缩期达峰时间的标准差(GC Time SD)。结果基本血流动力学指标HR(69.6±15.3)次/分、MAP(85.5±6.2)mmHg、SV(59.46±11.00)mL、CO(4.083±0.968)L/min、LVEF(56.82±7.76)%、RVEF(48.45±10.34)%。左心室应变与同步化指标:LS-ME4(28.2±7.1)%、LS-ME2(27.1±5.6)%、LS-ME3(24.6±3.5)%、GLS(26.6±3.7)%、GL Time SD(73.8±31.8)ms、MCS(23.2±4.4)%、GC Time SD(53.5±23.3)ms;右心室应变力与同步化指标:LS-RV(25.4±4.5)%,右心室同步化指标RV Time SD 44.0(34.8,64.0)ms。分析相关性得出LVEF与GLS呈正相关。结论基于2D-STI,使用Qlab软件可以客观评价动物猪模型心脏的心室应变力与同步化运动参数,分析得到健康猪TEE主要监测指标的生理范围,为进一步开展疾病模型研究提供了对比资料。Objective To establish an animal model for transesophageal echocardiography(TEE)monitoring cardiac function(including anesthesia method and ultrasound image acquisition),and analyze left ventricular and right ventricular myocardial strain and synchronous motor physiological parameters in swine heart and confidence interval of physiological parameters by two-dimensional speckle tracking imaging(2D-STI)and Qlab software,and analyze correlation between stroke volume(SV),left ventricular ejection fraction(LEVF)and left ventricular global longitudinal strain(GLS).Methods Ten healthy experimental swines were selected,aged 12-18 months with body mass of 45-60 kg.The heart rate(HR),mean arterial pressure(MAP),SV,cardiac output(CO),LVEF and right ventricular ejection fraction(RVEF)were recorded under general anesthesia.2D-STI was used for TEE,and indicators of TEE included left ventricular longitudinal stress(LS-ME4,LS-ME2,LS-ME3)and mean value GLS.The time to peak of longitudinal strain of each segment was recorded and its standard deviation(GL Time SD)was calculated;the standard deviation of right ventricular longitudinal strain(LS-RV)and time to peak at systole(RV Time SD)were calculated,the myocardial circumferential strain(MCS)and standard deviation of time to peak at systolic of left ventricular cir-cumferential strain(GC Time SD)were obtained by transthoracic echocardiography(TTE)in short-axis view of the left ventricle.Results The hemodynamic index:HR was(69.6±15.3)times/minutes,MAP was(85.5±6.2)mmHg,SV was(59.46±11.00)mL,CO was(4.083±0.968)L/minutes,LVEF was(56.82±7.76)%,RVEF was(48.45±10.34)%.The left ventricular strain and synchronization index:LS-ME4 was(28.2±7.1)%,LS-ME2 was(27.1±5.6)%,LS-ME3 was(24.6±3.5)%,GLS was(26.6±3.7)%,GL Time SD was(73.8±31.8)ms,MCS was(23.2±4.4)%,GC Time SD was(53.5±23.3)ms.The right ventricular strain and synchronization index:LS-RV(25.4±4.5)%;the right ventricular synchronization index:RV Time SD was 44.0(34.8,64.0)ms.The correlation analysis results showed that LVEF
