精神分裂症患者不同未治期与精神症状、认知功能及血脂的关系

摘　　要：目的:探讨不同未治时长的精神分裂症患者临床症状、认知水平及血脂的差异。方法:选取2017年11月至2021年4月于我院首次治疗的精神分裂症患者,以未治期(DUP)中位值(18个月)将患者分为长DUP(n=80)组和短DUP组(n=63)。收集患者一般人口学资料,评估阳性和阴性症状量表(PANSS)、成套神经心理状态测验(MCCB)、功能大体评定量表(GAF),并检测患者外周血脂蛋白水平。结果:短DUP患者组PANSS量表中的焦虑抑郁因子评分及GAF评分均高于长DUP患者,差异具有统计学意义(P<0.05);两组患者MCCB各项得分差异无明显的统计学意义(P>0.05),短DUP患者血高密度脂蛋白胆固醇(HDL-C)水平高于长DUP患者,差异具有统计学意义(P<0.05)。结论:未治期较短的精神分裂症患者有更明显的焦虑抑郁情绪,认知功能较长未治期损害较少,脂代谢紊乱较长DUP患者轻。Objective:To explore the differences of clinical symptoms,cognitive level and lipoprotein in schizophrenic patients with different duration of untreated psychosis(DUP).Method:Patients with schizophrenia who were first treated in our hospital from November 2017 to April 2021 were enrolled and divided into long DUP group(n=80)and short DUP group(n=63)by median time of DUP(18 months).The patients’general demographic data were collected and evaluated by Positive and Negative Symptom Scale(PANSS),Matrics Consensus Cognitive Battery(MCCB),General Functional Assessment Scale(GAF).Blood samples were taken to detect the level of peripheral lipoprotein.Results:The anxiety and depression factor score of PANSS and GAF Score in the short DUP group were higher than those in the long DUP group,the differences were statistically significant(P<0.05).There were no significant statistical differences in the scores of MCCB between the two groups(P>0.05).Serum high density lipoprotein cholesterol(HDL-C)levels of patients in short DUP group were higher than that of long DUP,and the difference was statistically significant(P<0.05).Conclusion:Schizophrenic with short untreated period have more obviously anxiety and depression,less cognitive deficiency,and less disturbance of lipid metabolism than patients with longer DUP.
