摘　　要：目的:探讨贝伐珠单抗(BEV)胸腔注射对肺腺癌恶性胸腔积液(MPE)患者血管内皮细胞生长因子(VEGF)、癌胚抗原(CEA)水平及免疫功能的影响。方法:选择2019年10月-2022年1月于新余市中医院治疗的88例肺腺癌MPE患者,按随机数字表法将其分为两组,各44例。对照组采用化疗联合BEV静脉输注治疗,观察组采用化疗联合BEV胸腔注射治疗。比较两组临床疗效,CEA、VEGF水平,免疫功能。结果:观察组疾病控制率(DCR)高于对照组(P<0.05);治疗后观察组CEA及VEGF水平均低于对照组(P<0.05);治疗后观察组CD3^(+)及CD4^(+)均高于对照组(P<0.05)。结论:肺腺癌MPE患者采用化疗、BEV胸腔注射联合治疗有利于降低VEGF、CEA水平,提高免疫功能,减少MPE。Objective:To investigate the effects of intrathoracic injection of Bevacizumab (BEV) on levels of vascular endothelial growth factor (VEGF),carcinoembryonic antigen (CEA) and immune function in patients with malignant pleural effusion (MPE) of lung adenocarcinoma.Method:A total of 88 patients with MPE of lung adenocarcinoma treated in Xinyu Hospital of Traditional Chinese Medicine from October 2019 to January 2022 were selected and divided into two groups according to random number table method,with 44 cases in each group.The control group was treated with chemotherapy combined with BEV intravenous infusion,and the observation group was treated with chemotherapy combined with BEV intrathoracic injection.The clinical efficacy,CEA and VEGF levels and immune function were compared between the two groups.Result:The disease control rate (DCR) in the observation group was higher than that in the control group (P<0.05).After treatment,CEA and VEGF levels in the observation group were lower than those in the control group (P<0.05).After treatment,CD3^(+)and CD4^(+)in the observation group were higher than those in the control group (P<0.05).Conclusion:Chemotherapy combined with intrathoracic injection of BEV in patients with MPE of lung adenocarcinoma is conducive to reducing the levels of VEGF and CEA,improving immune function,and reducing MPE.
