地西他滨联合小剂量CHG预激方案治疗老年急性髓系白血病的临床观察

摘　　要：目的分析地西他滨联合小剂量CHG预激方案[阿糖胞苷、高三尖杉酯碱、粒细胞集落刺激因子]对老年急性髓系白血病(AML)患者的治疗效果。方法抽取2015年1月至2021年12月许昌市中心医院收治的老年AML患者58例,按随机数字表法分为地西他滨组和CHG组,每组29例。CHG组予以小剂量CHG,地西他滨组予以地西他滨联合小剂量CHG。比较两组疗效、乳酸脱氢酶(LDH)、碱性磷酸酶(ALP)、血红蛋白(Hb)、血小板(PLT)、不良反应发生情况。结果地西他滨组总有效率(89.66%,26/29)高于CHG组(65.52%,19/29),P<0.05。治疗后,地西他滨组LDH、ALP水平低于CHG组(P<0.05),Hb、PLT水平高于CHG组(P<0.05)。地西他滨组不良反应发生率(13.79%,4/29)与CHG组(20.69%,6/29)比较差异未见统计学意义(P>0.05)。结论地西他滨联合小剂量CHG治疗AML患者,可提高疗效,改善患者血清酶水平及骨髓抑制情况,且联合用药安全性良好。Objective To analyze the effect of decitabine combined with inductive chemotherapy with low-dose of cytarabine,homoharringtonine and granulocyte colony-stimulating factor(CHG)on elderly patients with acute myeloid leukemia(AML).Methods A total of 58 elderly patients with AML treated in Xuchang Central Hospital from January 2015 to December 2021 were selected,and they were divided into decitabine group and CHG group by random number table method,with 29 cases in each group.The CHG group was given low-dose CHG,and the decitabine group was given decitabine combined with low-dose CHG.The efficacy,lactate dehydrogenase(LDH),alkaline phosphatase(ALP),hemoglobin(Hb),platelet(PLT),and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the decitabine group(89.66%,26/29)was higher than that of CHG group(65.52%,19/29),P<0.05.After treatment,levels of LDH and ALP in the decitabine group were lower than those in the CHG group(P<0.05),and levels of Hb and PLT in the decitabine group were higher than those in the CHG group(P<0.05).There was no significant difference in the incidence of adverse reactions between the decitabine group(13.79%,4/29)and the CHG group(20.69%,6/29),P>0.05.Conclusions Decitabine combined with low-dose CHG in the treatment of elderly patients with AML can improve the efficacy,improve serum enzyme level and bone marrow suppression,with great safety.
