腭侧与颊侧微种植体支抗远移上颌磨牙的临床疗效及对侧貌美学效果的影响研究

摘　　要：目的:探讨腭侧与颊侧微种植体支抗远移上颌磨牙的临床疗效及对侧貌美学效果的影响。方法:选取2018年1月-2020年12月诊治的非拔牙不完全Ⅱ类关系错(牙合)畸形患者98例,将患者随机分为腭侧组与颊侧组,每组49例,两组分别采取腭侧与颊侧植入微种植体支抗行正畸治疗,观察患者植入成功率,治疗前和治疗后6个月时上颌磨牙远移效果及侧貌美学标志点变化。结果:腭侧组共植入116枚微种植钉,种植成功率为89.66%,颊侧组共植入108枚微种植钉,植入成功率为79.66%,两组植入成功率比较差异有统计学意义(P<0.05);两组治疗后翼上颌-上第一恒磨牙(Ptm-6)值较治疗前明显改善(P<0.05),且治疗后腭侧组Ptm-6值改善程度优于颊侧组(P<0.05);两组患者治疗后较治疗前鼻唇角、面型角、上唇突度明显改善,差异比较有统计学意义(P<0.05),下唇突度治疗前后测量值比较差异无统计学意义(P>0.05)。结论:使用微种植体支抗远移磨牙时采取腭侧入路植入较颊侧颊侧入路植入成功率高,远移效果更好,软组织侧貌美学均较治疗前得到改善。Objective To investigate effect of he clinical effect and aesthetics of lateral appearance on palatal and buccal implantation of microimplant anchorage of maxillary molars,and provide reference for orthodontic treatment of maxillary molars.Methods The 98 cases patients with non-tooth extraction incomplete categoryⅡmalocclusion from January 2016to December 2018,patients were randomly divided into palatal group and buccal group,49 cases in each group,the two groups were treated with palatal and buccal microimplant screws,palatal and buccal microimplant nails were implanted for orthodontic treatment,to looking at the success rate of implantation,the effect of distal displacement of maxillary molars and the changes of esthetic markers in aesthetics of lateral appearance at 6 months before and after treatment.Results 116 micro-implant nails were implanted in the palatal group,the success rate of 89.66%,108 micro-implant nails were implanted in the buccal group,the success rate of 89.66%,the difference was statistically significant(P<0.05).Ptm-6 value of the first permanent molar of the upper wing maxilla was significantly improved after treatment in two groups(P<0.05),and ptm-6 value in the palatal group was better than that in the buccal group(P<0.05).The nasolabial horn,facial Angle and upper lip protrusion were significantly improved in the two groups,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the measured values of lower lip protrusion before and after treatment(P>0.05).Conclusion The success rate of the palatal approach was higher than that of the buccal approach when the microimplant anchorage was used for distant molars,the remoteness effect was better,and the soft tissue profile aesthetics was better than before treatment.
