摘 要:目的探讨牙周基础治疗对糖尿病型牙周炎患者的龈下菌群变化及糖代谢的临床价值。方法选取2020年1月—2022年2月龙岩市第一医院口腔科收治的140例糖尿病型牙周炎患者为研究对象,将其分为对照组(70例,接受常规治疗)和干预组(70例,在对照组基础上接受牙周基础治疗)。比较两组患者干预前、干预后90d的龈下菌群变化及糖代谢情况。结果干预组90d后空腹血糖(FPG)、糖耐量试验(OGTT)和糖化血红蛋白Hb A1c水平均高于对照组,差异有统计学意义(P<0.05)。干预组治疗90d后龈卟啉单胞菌(Pg)、中间普氏菌(Pi)、福赛坦氏菌(Tf)、伴放线放线杆菌(Aa)检测水平均低于对照组,差异有统计学意义(P<0.05)。两组患者治疗90d后龈下菌群及糖代谢指标相较治疗前均有改善。Hb A1c水平与Pg、Pi、Tf、Aa数量呈正相关(r=0.929、0.880、0.903、0.876,P<0.05)。结论牙周基础治疗能有效改善糖尿病型牙周炎患者的龈下菌群平衡,纠正糖代谢异常,在糖尿病和牙周病预防治疗中具有科学性临床价值。Objective To investigate the clinical value of basic periodontal treatment on the changes of subgingival flora and glucose metabolism in cases of diabetic-type periodontitis.Methods 140 diabetic periodontitis patients in the Department of Stomatology of Longyan First Hospital from January 2020 to February 2022 were selected as the research subjects,they was divided into the control group(70 cases,conventional treatment)and the intervention group(70 cases,basic periodontal treatment based on the control group).The changes of subgingival flora and glucose metabolism before and 90 d after the intervention were compared between the two groups of patients.Results Fasting blood glucose(FPG),glucose tolerance test(OGTT)and glycated hemoglobin Hb A1c levels after 90 d treatment in the intervention group were higher than those in the control group,the difference was statistically significant(P<0.05).Porphyromonas gingivalis(Pg),Proteus intermedius(Pi),Prevotella intermedia(Tf),and Actinobacillus actinomycetemcomitans(Aa)detection levels in the intervention group were lower than the control group,the difference was statistically significant(P<0.05).The subgingival microflora and glucose metabolism index improved after 90 d of treatment compared with before the treatment.The Hb A1c levels were positively correlated with the numbers of Pg,Pi,Tf,and Aa(r=0.929,0.880,0.903,0.876,P<0.05).Conclusion Periodontal basic treatment can effectively improve the balance of subgingival flora and correct abnormal glucose metabolism in cases of diabetic periodontitis,and has scientific clinical value in the preventive treatment of diabetes and periodontal disease.
