摘 要:目的从年龄分层角度分析幽门螺杆菌(Hp)感染相关影响因素,并探讨关键影响因素的作用。方法回顾性分析2021年1月1日至12月31日在浙江中医药大学附属第三医院接受健康体检人群的临床资料,依据14 C尿素呼气检测结果将体检者分为Hp感染组和无Hp感染组,采用logistic回归分析探究影响Hp感染率的因素,分析不同年龄人群Hp感染的影响因素。结果纳入的1168例体检者中,Hp阳性者395例(33.82%),男性Hp感染率为36.35%(277/762),女性为29.06%(118/406),差异有统计学意义(P<0.05)。Hp感染组年龄(t=3.124,P<0.01)、BMI(t=2.773,P<0.01)、PFG(t=2.676,P<0.01)、尿酸(t=2.627,P<0.01)均显著高于无Hp感染组,差异均有统计学意义。Logistic回归分析发现,年龄、BMI、FPG与Hp感染均相关(均P<0.05)。年龄增加1岁、BMI增加1kg/m2、FPG增加1mmol/L,Hp感染的风险分别增加10.18%、10.49%和11.02%。FPG正常人群Hp阳性率随年龄增大呈逐渐上升趋势。31~40岁和41~50岁人群中,FPG偏高人群Hp阳性率显著高于FPG正常人群(P<0.05)。FPG和BMI均偏高时,31~40岁人群Hp阳性率最高。结论年龄、BMI和FPG是Hp感染的独立危险因素。随着年龄增加,Hp感染的发生率逐步增加。BMI和FPG可能是31~40岁人群Hp感染的关键因素。Objective The risk factors of Helicobacter pylori(Hp)infection and their relation with ages.Methods A total of 1168 subjects who underwent health check-up in the Third Affiliated Hospital of Zhejiang Chinese Medical University between January and December 2021 were enrolled.According to results of the 14C urease breath test,the participants were divided into Hp-infected and non-Hp-infected groups,and the risk factors of Hp infection were analyzed by logistic regression analysis.Results The Hp-positive rate was 33.82%among 1168 participants,36.35%(277/762)for males and 29.06%(118/406)for females(P<0.05).Age(t=3.124,P<0.01),BMI(t=2.773,P<0.01),FPG(t=2.676,P<0.01),and uric acid(t=2.627,P<0.01)were significantly higher in Hp-positive group than those in Hp-negative group.Logistic regression analysis revealed that age,BMI,and FPG were independent risk factors of Hp infection(P<0.05).One year increase in age,1 kg/m2 increase in BMI,and 1 mmol/L increase in FPG increased the risk of Hp infection by 10.18%,10.49%,and 11.02%,respectively.In addition,the Hp-positive rate in those with normal FPG tended to increase with age,the Hp-positive rate in those with high FPG was significantly higher than that in those with normal FPG in the age groups of 31-40 and 41-50 years(P<0.05).The Hp-positive rate in the age group of 31-40 years was the highest when both FPG and BMI were high.Conclusion The study shows that age,BMI and FPG are risk factors for Hp infection;the infection rate increases with the increasing age and BMI and FPG may be the main factors related the Hp infection in those aged 31 to 40.
