二甲硅油散和纯净水联用提高内镜下可见度和超声内镜图像质量的效果研究

摘　　要：目的评价检查前联合饮用二甲硅油溶液和纯净水提高内镜下胃黏膜可见度和超声内镜图像清晰度的效果。方法选取120例经内镜检查确诊存在胃黏膜隆起需进一步行超声内镜检查的患者作为研究对象,将其随机分为观察组和对照组,每组60例。两组检查前均禁食、禁饮时间>6 h,对照组术前15~20 min服用二甲硅油溶液20 mL,观察组术前40~60 min饮用纯净水总量约400 mL,术前15~20 min服用二甲硅油溶液20 mL。比较两组患者内镜下胃黏膜可见度、超声内镜图像清晰度、不良反应发生情况和患者满意度。结果观察组患者内镜下胃黏膜可见度情况、超声内镜胃腔和胃黏膜表面清晰度、满意度情况均优于对照组(均P<0.05)。两组均无严重呕吐、误吸、反流、窒息等不良反应发生。结论超声内镜检查前(胃排空时间允许范围内)饮用适量纯净水,可以有效地消除患者胃内黏液和泡沫;二甲硅油散和纯净水联用可提高内镜下胃黏膜可见度、超声内镜图像清晰度和患者满意度。Objective To evaluate the effect of drinking of dimethicone solution combined with purified water before examination on improving endoscopic gastric mucosal visibility and echoendoscopic image clarity.Methods A total of 120 patients diagnosed with gastric mucosal protrusion by endoscopy requiring further echoendoscopy were selected as the study subjects,and they were randomly divided into an observation group or a control group,with 60 cases in each group.The two groups were performing food-and water-deprivation for more than 6 h before the examination,the control group took 20 mL dimethicone solution 15~20 min before surgery,the observation group drank about 400 mL purified water 40~60 min before surgery,and 20 mL dimethicone solution 15~20 min before surgery.The endoscopic gastric mucosal visibility,echoendoscopic image clarity,incidence of adverse reactions and patients′satisfaction were compared between the two groups.Results The endoscopic gastric mucosal visibility,the echoendoscopic clarity of gastric cavity and gastric mucosal,and satisfaction in the observation group were better than those in the control group(all P<0.05).There were no adverse reactions such as severe vomiting,aspiration,reflux,and asphyxia in either group.Conclusion Drinking an appropriate amount of purified water before echoendoscopy(within the allowable time for gastric emptying)can effectively eliminate mucus and foam in patients′stomach.The combination of dimethicone powder and purified water can improve endoscopic mucosal visibility,echoendoscopic image clarity,and patient′s satisfaction.
