摘　　要：目的:探究Nd∶YAP激光辅助牙周基础治疗对Ⅲ期B/C级牙周炎的疗效及龈沟液中低氧诱导因子-1α(HIF-1α)、细胞间黏附分子-1(ICAM-1)水平的变化。方法:采用分口实验,在26例Ⅲ期B/C级牙周炎患者的对照组行牙周基础治疗,实验组在基础治疗后辅助使用Nd∶YAP激光。在治疗前、治疗后6周及3月记录PD、BOP,并用纸尖法收集龈沟液,使用ELISA试剂盒检测龈沟液中HIF-1α、ICAM-1的水平。结果:与基线相比,治疗后6周及3月时两组的PD、BOP(%)及龈沟液中HIF-1α、ICAM-1水平均显著降低(P<0.05),实验组治疗后6周的PD、BOP(%)及治疗后3月的PD、ICAM-1浓度显著低于对照组(P<0.05)。结论:Nd∶YAP激光辅助牙周基础治疗在短期内对Ⅲ期B/C级牙周炎患者安全有效,能降低PD、BOP(%)及龈沟液中ICAM-1水平。Objective:To explore the curative effect of Nd∶YAP laser treatment as an adjuvant therapy for periodontal initial therapy in patients with stageⅢ,grade B/C periodontitis and the levels of hypoxia inducible factor-1α(HIF-1α)and intercellular adhesion molecule-1(ICAM-1)in gingival crevicular fluid(GCF).Methods:Clinical periodontal parameters of PD and BOP in 26 patients with stage Ⅲ,grade B/C periodontitis were recorded at baseline.GCF samples were collected and all teeth received periodontal initial therapy.Using a split mouth study design,the teeth in test group was additionally treated with an adjuvant therapy of Nd∶YAP laser.The same examination was performed 6 weeks and 3 months after operation.Then,the levels of HIF-1α and ICAM-1 were detected using ELISA kit.Results:At 6 weeks and 3 months postoperatively,PD,BOP,the concentrations of HIF-1αand ICAM-1 in GCF were significantly reduced(P<0.05).PD at 6 weeks and 3 months,rate of BOP at 6 weeks and the concentrations of ICAM-1 at 3 months in the test group were lower than those in the control group(P<0.05).Conclusion:Nd∶YAP laser combined with periodontal initial therapy is safe and effective for the patients with stage Ⅲ,grade B/C periodontitis in a short term,and can significantly reduce PD,rate of BOP and ICAM-1 in GCF.
