摘　　要：目的调查流浪精神分裂症出院患者药物使用情况。方法采取回顾性调查方法,通过电子病历系统调查2016年1月1日至2021年9月30日上海市精神卫生中心闵行院区流浪精神分裂症出院患者的药物使用情况,客观填写自制的信息调查表,对出院患者用药情况进行分析。结果490例流浪精神分裂症患者出院带药以单一用药为主(96.5%),排名前三为利培酮、奥氮平、氯氮平,其中有3例患者奥氮平超剂量使用。使用的典型抗精神病药物仅为舒必利,占6.9%,其余均使用非典型抗精神病药物。近三年来,用药种类呈多样化趋势,联合用药均为非典型抗精神病药物联用,联合用药和合并其他精神类药物比例均上升(P<0.05)。合并通便药物最常见(25.7%)。结论流浪精神分裂症患者出院带药规范合理,以新型抗精神病药物治疗为主,服用抗精神病药物期间要注意监测和评估患者便秘情况。Objective To investigate the drug use status of discharged patients with vagrant schizophrenia in our hospital.Methods A retrospective study was performed to investigate the drug use status in discharged patients with vagrant schizophrenia who were treated in our hospital from January 1,2016 to September 30,2021 through electronic medical record system.The self-made questionnaire was used for data collection and analysis.Results The 96.5%of the 490 patients were treated by single medication after discharging from hospital,and the top three drugs were risperidone,olanzapine and clozapine.Moreover olanzapine was overdosed in 3 patients,and the typical antipsychotic drug was sulpiride,accounting for 6.9%,the rest ones were atypical antipsychotics.In recent three years,the types of drug use showed a trend of diversification,the combination therapy was mainly the combination of atypical antipsychotic drugs,and the proportion of combination therapy and combination of other psychotropic drugs was gradually increased.Combination of defecation drugs was the most common(25.7%).Conclusion It is relatively reasonable in the drug use for discharged patients with vagrant schizophrenia in our hospital,who are treated mainly by novel antipsyhotics.It is necessary to pay much attention to the monitoring of drug use and the constipation status of patients.
