喹硫平联合安理申对卒中后认知障碍患者的认知功能疗效及血液指标分析

摘　　要：目的探究喹硫平联合安理申治疗对卒中后认知障碍患者认知功能以及血清学指标的改善效果。方法选取2019年12月—2020年7月期间河南大学第一附属医院收治的卒中后认知障碍患者共78例,采用随机数表法将上述患者分为观察组(n=39)以及对照组(n=39);对照组接受安理申治疗;观察组接受喹硫平联合安理申治疗;记录并比较两组治疗前后简明精神状态检查量表(MMSE)、神经功能缺损评分(NIHSS)以及日常生活活动能力(ADL)评分、胰岛素样生长因子-1(IGF-1)、细胞间黏附分子-1(ICAM-1)以及内皮素(ET)水平、用药后不良反应发生率。结果治疗后,MMSE评分以及ADL评分均升高,观察组高于对照组(P<0.05);NIHSS评分则出现降低,观察组低于对照组(P<0.05);治疗后,两组IGF-1相比治疗前升高,观察组高于对照组;ICAM-1以及ET相比治疗前下降,观察组低于对照组,差异均有统计学意义(P<0.05);另外,两方案不良反应发生率无明显差别(P>0.05)。结论喹硫平联合安理能够有效改善脑卒中后认知障碍患者的精神状态以及认知功能、提升生活质量、调节神经系统相关血液指标,且具有良好的安全性。Objective To explore the improvement effect of quetiapine combined with aricept on cognitive function and blood indexes in patients with post-stroke cognitive disorders.Methods A total of 78 patients with post-stroke cognitive disorders admitted to the hospital were enrolled between December 2019 and July 2020.According to random number table method,they were divided into observation group(n=39)and control group(n=39).The control group was treated with aricept,while observation group was additionally treated with quetiapine.The scores of mini-mental state examination(MMSE),National Institutes of Health Stroke Scale(NIHSS)and activity of daily living(ADL),levels of insulin-like growth factor-1(IGF-1),intercellular adhesion molecule-1(ICAM-1)and endothelin(ET)before and after treatment,and incidence of adverse reactions after medication in the two groups were recorded and compared.Results After treatment,MMSE and ADL scores in both groups were increased,which were higher in observation group than control group(P<0.05).After treatment,NIHSS scores in both groups were decreased,which were lower in observation group than control group(P<0.05).After treatment,IGF-1 in both groups was increased,which was higher in observation group than control group,while levels of ICAM-1 and ET in both groups were decreased,which were lower in observation group than control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Quetiapine combined with aricept can effectively improve mental state,cognitive function and quality of life in patients with post-stroke cognitive disorders,and regulate blood indexes related to nervous system.The combined medication will not increase the incidence of adverse reactions,with good safety.
