多发结节型脾脏窦岸细胞血管瘤的影像表现与病理对照分析

摘　　要：目的:总结分析脾脏窦岸细胞血管瘤(LCA)的临床特点、影像表现及病理特征,提高对本病的认识。方法:回顾性分析6例多发结节型LCA患者的临床及影像表现,并与病理进行对照分析。结果:6例LCA患者中3例因腹部不适就诊,4例伴有不同程度贫血。较大病灶(直径>2.0 cm)强化方式为由内向外,出现“花瓣样”改变,可能是其特征性表现;较小病灶(直径<2.0 cm)强化方式显示欠清。T_(2)WI上的“雀斑征”、DWI高信号是LCA的特征性表现。结论:LCA的CT、MRI表现具有一定特征性,“花瓣样”强化特点、T_(2)WI上的“雀斑征”结合DWI高信号可增加诊断信心,确诊需依靠病理免疫组化检查。Objective:To summarize the clinical manifestations,imaging findings and pathological features of littoral cell angioma(LCA)of spleen.Methods:The clinical data and imaging findings of 6 patients with multiple nodular LCA were retrospectively analyzed and correlated with pathology.Results:Among six LCA patients,3 had abdominal discomfort,and 4 were associated with anemia of varying degrees.For lesions greater than 2.0cm,multiple LCA enhanced from inside to periphery,with“petal-like”changes;however,lesions smaller than 2.0cm showed indeterminate enhancement.The imaging characteristics of LCA were spotted hypointensity on T_(2)WI and hyperintensity on DWI.Conclusion:LCA have certain CT and MRI characteristics.The"petal-like"enhancement features,spotted hypointensity on T_(2)WI and hyperintensity on DWI can increase the diagnostic confidence.The definite diagnosis depends on pathological immunohistochemical examination.
