无托槽隐形矫治器推磨牙向远中矫治骨性Ⅱ类错牙合的效果及对正面微笑美学的影响

摘　　要：目的 分析无托槽隐形矫治器推磨牙向远中矫治骨性Ⅱ类错牙合的效果及其对正面微笑美学的影响。方法 回顾性分析2019年6月至2020年6月期间到我院接受正畸治疗的76例骨性Ⅱ类错牙合畸形患者临床资料,根据矫治方法不同将病例分为2组,对照组(n=36)使用直丝弓固定矫治器,观察组(n=36)使用无托槽隐形矫治器矫治,比较两组推磨牙向远中效率、矫治前后PAR指数、矫治后疼痛评分以及矫治前后微笑时唇部形态变化。结果 观察组上颌第一、第二磨牙移位表达率显著高于对照组,且磨牙远移移动速度显著优于对照组(P<0.05)。矫治后,两组患者的PAR指数各维度评分均较治疗前明显减少,且观察组的PAR指数评分显著少于对照组(P<0.05)。观察组患者矫治后1、3、5、7dVAS评分少于对照组(P<0.05)。矫治后6个月,观察组患者微笑时的颊旁间隙、微笑弧、微笑指数显著优于对照组(P<0.05)。结论 应用无托槽隐形矫治器能够有效推磨牙向远中,减轻患者疼痛感,将其应用于骨性Ⅱ类错牙合矫治中,既可提高疗效,又可以改善患者的微笑美观,值得推广应用。Objective To explore the effect of using the invisible appliance without brackets to push the molar to the distal orthodontic treatment of bony Ⅱ type malocclusion and its influence on the aesthetic of positive smile.Methods Retrospective analysis of the clinical data of 76 patients with skeletal class Ⅱ malocclusion who received orthodontic treatment in our hospital from June 2019to June 2020.The cases were divided into two groups according to different treatment methods.The control group(n=36) used straight wire arch fixed appliance,and the observation group(n=36) used non bracket invisible appliance.The distal molar pushing efficiency,PAR index before and after treatment,pain score after treatment and changes in lip morphology during smiling were compared between the two groups.Results In the observation group,the distal pushing molar efficiency was 92.11%,and the expected molar displacement expression rate was 86.84%,which were signfficantly higher than the control group(P<0.05).After correction,the PAR index scores of all dimensions in 2 groups were significantly decreased compared with before treatment,and the PAR index scores of observation group were significantly lower than control group(P<0.05).The VAS scores of observation group were lower than control group(P<0.05).The buccal space,smile arc and smile index in observation group were significantly better than control group(P<0.05).Conclusion Invisible appliance without bracket can effectively push molar to the far center and reduce the pain of patients.It can be used in the orthodontic treatment of bony Ⅱ malocclusion,which can not only improve the efficacy,but also improve the beauty of the smile of patients.
