视神经脊髓炎谱系疾病急性期患者脑脊液IL-6及IL-10的表达

摘　　要：目的检测视神经脊髓炎谱系疾病(NMOSD)急性期患者脑脊液中白细胞介素6(IL-6)及白细胞介素10(IL-10)水平,观察IL-6与IL-10在NMOSD急性期患者脑脊液中的表达。方法选取AQP4抗体阳性的急性期NMOSD患者(NMOSD组)10例,13例年龄、性别匹配的健康者为对照组,应用ELISA方法测定10例急性期NMOSD患者及13健康对照者脑脊液中IL-6及IL-10的水平。结果与对照组比较,NMOSD组脑脊液中IL-6水平升高,2组比较差异有统计学意义(P=0.01);NMOSD组脑脊液IL-10水平降低,2组比较差异无统计学意义(P=0.33)。首发症状视神经炎的急性期NMOSD患者脑脊液IL-6水平较首发症状急性脊髓炎的急性期NMOSD患者高,差异有统计学意义(P=0.03);急性脊髓炎和视神经炎患者脑脊液IL-10水平比较差异无统计学意义(P=0.58)。不同病程急性期NMOSD患者脑脊液IL-6、IL-10水平差异无统计学意义(P<0.05)。脑脊液IL-6水平与急性期NMOSD患者扩展残疾状态量表(EDSS)评分无显著相关性(P=0.49),脑脊液IL-10水平与急性期NMOSD患者EDSS评分有显著相关(P=0.01)。结论急性期NMOSD患者中IL-6水平升高并促进分泌自身抗体,发挥致病作用,IL-10也可能参与NMOSD发病过程。Objective To detect the values and concentrations of interleukin-6(IL-6)and interleukin-10(IL-10)in the cerebrospinal fluid of patients with optic neuromyelitis spectrum disease(NMOSD)in the acute phase,and observe the expressions of IL-6 and IL-10 in the cerebrospinal fluid of patients with NMOSD.Methods Ten patients with acute NMOSD who had positive AQP4 antibody and 13 healthy controls(HCs)with age and sex matching were selected.ELISA determined the cerebrospinal fluid IL-6 and IL-10 in 10 patients with acute NMOSD and 13 healthy controls.Results Compared with the healthy control group,the level of IL-6 in the cerebrospinal fluid of 10 patients in the acute NMOSD group increased,and the difference between the two groups was statistically significant(P=0.003).The level of cerebrospinal fluid IL-10 decreased in the NMOSD group,but the difference between the two groups was not statistically significant(P=0.335).The level of cerebrospinal fluid IL-6 in NMOSD patients with the first symptom of acute optic neuritis was higher than that in NMOSD patients with the first symptom of acute myelitis,and the difference of cerebrospinal fluid IL-6 was statistically significant(P=0.030).There was no significant difference in cerebrospinal fluid IL-10 between acute myelitis and optic neuritis(P=0.585).There was no significant difference between the course of disease and cerebrospinal fluid IL-6 and IL-10 levels in patients with acute NMOSD.There was no significant correlation between cerebrospinal fluid IL-6level and EDSS score in patients with acute NMOSD(r=0.247,P=0.491).There was a significant correlation between cerebrospinal fluid IL-10 level and EDSS score in patients with acute NMOSD(r=0.739,P=0.015).Conclusion In patients with acute NMOSD,the level of IL-6 increases and promotes the secretion of autoantibodies,playing a pathogenic role.IL-10 may also be involved in the pathogenesis of NMOSD.
