摘　　要：目的 探讨围绝经期女性发生缺血性脑卒中的危险因素。方法 选取自2021年1月至2022年1月于锦州医科大学附属第一医院神经内科住院治疗的50例围绝经期女性且首次发生缺血性脑卒者为观察组,并选取同期进行体检的50例健康围绝经期女性为健康组。收集并比较两组年龄,吸烟情况,合并高血压、糖尿病、冠心病情况,月经相关情况[是否绝经、绝经年龄、月经初潮年龄],体质量指数(BMI),实验室检查相关指标[低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、小而密低密度脂蛋白胆固醇(sdLDL-C)、尿酸(UA)、中性粒细胞百分比、残余胆固醇(RC)、同型半胱氨酸(HCY)、空腹血糖、甘油三酯(TG)及总胆固醇],以及颈动脉内中膜厚度(IMT)等。采用多因素Logistic回归模型分析围绝经期女性缺血性脑卒中的可能危险因素。结果 两组高血压史、冠心病史、绝经年龄、月经初潮年龄、sdLDL-C、UA、RC、IMT比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,高血压、月经初潮年龄、绝经年龄、IMT为围绝经期女性发生缺血性脑卒中的独立危险因素(P<0.05)。结论 高血压、月经初潮年龄、绝经年龄、IMT为围绝经期女性发生缺血性脑卒中的独立危险因素。临床工作中,应关注围绝经期女性这一特殊群体,针对其危险因素进行有效的干预和防治。Objective To investigate the risk factors of ischemic stroke in perimenopausal women.Methods A total of 50 perimenopausal women with ischemic stroke for the first time who were hospitalized in the Department of Neurology, the First Affiliated Hospital of Jinzhou Medical University from January 2021 to January 2022 were selected as the observation group, and 50 healthy perimenopausal women who underwent physical examination during the same period were selected as the healthy group.The age,smoking,hypertension,diabetes,coronary heart disease,menstruation related status[menopause,age of menopause,age of menarche],body mass index(BMI),laboratory examination[low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),small dense low-density lipoprotein cholesterol(sdLDL-C),uric acid(UA),neutrophil percentage,remnant cholesterol(RC),homocysteine(HCY),fasting blood glucose,triglyceride(TG) and total cholesterol],and carotid intimamedia thickness(IMT) were collected and compared between the two groups.Multivariate Logistic regression model was used to analyze the possible risk factors of ischemic stroke in perimenopausal women.Results There were statistically significant differences in hypertension history,coronary heart disease history,age of menopause,age of menarche,sdLDL-C,UA,RC,and IMT between the two groups(P <0.05).Multivariate Logistic regression analysis showed that hypertension,age of menarche,age of menopause,and IMT were independent risk factors for ischemic stroke in perimenopausal women(P <0.05).Conclusion Hypertension,age of menarche,age of menopause,and IMT are independent risk factors for ischemic stroke in perimenopausal women.In clinical work,attention should be paid to perimenopausal women,this unique group,according to their risk factors for effective intervention and prevention.
