摘 要:目的 使用锥形束CT(CBCT)研究翼腭管的解剖及其与周围结构的关系,为临床LeFort Ⅰ型截骨术提供指导。方法 从CBCT数据库中随机选取30例正常成年人颌面部CBCT数据,比较左右侧各测量标志点数值之间的差异。结果上颌窦内侧壁从犁状孔边缘至翼腭管的平均距离是35.25 mm。颧牙槽嵴至翼腭管25.41 mm,翼腭管至中线16.68 mm,犁状孔边缘至中线的距离是13.87 mm,骨性鼻中隔长度是46.27 mm。犁状孔边缘至翼腭管连线与矢状面的夹角为6°14′。比较左右侧各测量标志点数值之间的差异,差异无统计学意义(P>0.05)。结论 使用CBCT测量LeFort Ⅰ型截骨术相关的翼腭管的解剖数据准确、有效,可为避免损伤腭降血管等结构提供临床指导。Objective Conical beam CT(CBCT)was used to study the anatomy of pterygopalatine canal and its relationship with the surrounding structure, so as to provide guidance for clinical Lefort Ⅰ osteotomy.Methods A total of 30 normal adult maxillofacial CBCT data were randomly selected from CBCT database to compare the difference of the values of the measured landmarks on the left and right sides.Results At the level of LeFort Ⅰ osteotomy, the average distance of the inner wall of the maxillary sinus from the edge of the plow-shaped hole to the pterygopalatine canal was 35.25 mm. The zygomatic alveolar ridge to the pterygopalatine canal is 25.41 mm, the pterygopalatine canal to the midline is 16.68 mm, the distance from the edge of the plow-shaped foramen to the midline is 13.87 mm, and the length of the bony nasal septum is 46.27 mm. The angle between the edge of the plow hole and the pterygopalatine canal and the sagittal plane is 6°14′. Compare the difference between the values of the measured markers on the left and right sides.The difference was not statistically significant(P>0.05).Conclusion The use of CBCT to measure the anatomical data of the pterygopalatine canal associated with LeFort Type Ⅰ osteotomy is accurate and effective, and can provide clinical guidance for avoiding damage to the descending palatine vessels and other structures.
