消斑汤治疗高危非致残性缺血性脑血管病痰热证的临床观察

摘　　要：目的:研究“消斑汤”治疗痰热型高危非致残性缺血性脑血管病(HR-NICE)的临床疗效。方法:选取80例痰热型HR-NICE患者作为研究对象,分为消斑汤组(40例)与常规治疗组(40例),常规治疗组组采用脑血管病二级预防治疗;消斑汤组在常规治疗组的基础上加用“消斑汤”,两组治疗周期同为3个月。比较两组治疗前后相关血清学指标(甘油三酯、高密度脂蛋白、总胆固醇、低密度脂蛋白、D二聚体)、生活能力评分、NIHSS评分的变化。结果:消斑汤组和常规治疗组在甘油三酯、低密度脂蛋白、总胆固醇上均较治疗前数值有所下降(P<0.05);在改善生活能力评分方面,治疗后两组患者生活能力评分数值均升高(P<0.05);在改善NIHSS评分方面,治疗后消斑汤组NIHSS评分有所下降(P<0.01),常规治疗组在NIHSS评分上亦有下降(P<0.05)。消斑汤组和常规治疗组进行组间比较,消斑汤组在总胆固醇、低密度脂蛋白、甘油三酯上所降数值均比常规治疗组幅度更大(P<0.05);消斑汤组的生活能力评分较常规治疗组数值升高更为明显(P=0.0010<0.01);消斑汤组的NIHSS评分较常规治疗组数值降低幅度更大(P=0.0081<0.01)。结论:“消斑汤”可以改善痰热型HR-NICE患者的相关血清学指标(总胆固醇、低密度脂蛋白、甘油三酯)、生活能力评分、NIHSS评分。Objective To study the clinical efficacy of"Xiaoban Decoction"in the treatment of high risk non-disabling ischemic cerebrovascular disease(HR-NICE)with phlegm-heat type.Methods Selection of jiangxi university of traditional Chinese medicine affiliated hospital treated 80 cases of patients with type phlegm heat HR-NICE as the research object,is divided into extinction soup group(40 cases)and conventional treatment group(40 cases)and conventional treatment way using cerebrovascular disease secondary prevention treatment,eliminate soup group on the basis of routine treatment group,plus use soup with two groups of treatment cycle for 3 months.The changes of related serological indexes(triglyceride,high density lipoprotein,total cholesterol,low density lipoprotein,D dimer),living ability score and NIHSS score were compared between 2 groups before and after treatment.Results Triglyceride,low density lipoprotein and total cholesterol in Xiaoban Decoction group and conventional treatment group were decreased compared with those before treatment(P<0.05);In terms of improving living ability score,the value of living ability score in both groups increased after treatment(P<0.05);In terms of improving NIHSS score,NIHSS score of xiaoban Decoction group decreased after treatment(P<0.01),and NIHSS score of conventional treatment group also decreased(P<0.05).Compared with the conventional treatment group,the reduction of total cholesterol,low density lipoprotein and triglyceride in the Xiaoban Decoction group was larger than that in the conventional treatment group(P<0.05).The living ability score of xiaoban Decoction group increased more obviously than that of conventional treatment group(P=0.0010<0.01).The NIHSS score of xiaoban Decoction group decreased more significantly than that of conventional treatment group(P=0.0081<0.01).Conclusion“Xiaoban Decoction”can improve the related serological indexes(total cholesterol,low density lipoprotein,triglyceride),living ability score and NIHSS score of patients with phlegm-heat TY
