2016-2018年安徽省恶性肿瘤住院患者直接医疗费用分析

摘　　要：目的分析恶性肿瘤住院患者的直接医疗费用情况及其影响因素,为合理控制肿瘤患者医疗费用提供科学依据。方法收集2016-2018年安徽省12个县区(当涂县、繁昌区、肥东县、蒙城县、谯城区、太和县、天长县、望江县、砀山县、灵璧县、寿县和颍东区)恶性肿瘤住院患者直接医疗费用信息合计212176条。使用均数和中位数描述10种主要恶性肿瘤的直接医疗费用。通过非参数检验和多元线性回归分析恶性肿瘤患者直接医疗费用及其影响因素。结果2016-2018年安徽省10种常见恶性肿瘤住院患者例均直接医疗费用位居前3的依次为白血病(50472.26元)、结直肠癌(30083.01元)和食管癌(25962.01元);次均直接医疗费用位居前3的依次为白血病(20535.03元)、肝癌(14324.14元)和食管癌(13286.15元);日均直接医疗费用位居前3的依次为甲状腺癌(1294.90元)、肝癌(1179.49元)和结直肠癌(1155.98元)。非参数检验结果显示,性别、年龄、医院等级、医院类型和住院天数都是影响恶性肿瘤患者次均直接医疗费用的影响因素。标准化回归系数显示对次均直接医疗费用的影响较大的是住院天数、三级医院、二级医院和二类医院。结论安徽省恶性肿瘤患者疾病负担较重,医院等级、住院天数、医院类型和恶性肿瘤种类是影响恶性肿瘤患者次均直接医疗费用的主要因素,相关部门应协同合作,以降低恶性肿瘤患者的经济负担。Objective To analyze the direct medical expenses of inpatients with malignant tumors and its influencing factors,and provide a scientific basis for reasonable control of medical expenses of cancer patients.Methods The information about direct medical expenses of inpatients with malignant tumor in 12counties in Anhui Province from 2016to 2018was collected.Means and medians were used to describe direct medical costs for 10major malignancies.Nonparametric test and multiple linear regression were used to analyze the direct medical expenses of patients with malignant tumor and the influencing factors.Results From 2016to 2018,the direct medical expenses of 10common malignant tumor inpatients in Anhui Province ranked top 3in order:leukemia(50472.26yuan),colorectal cancer(30083.01yuan),esophageal cancer(25962.01yuan).The top 3direct medical expenses were leukemia(20535.03yuan),liver cancer(14324.14yuan),esophageal cancer(13286.15yuan).The top 3daily direct medical expenses were thyroid cancer(1294.90yuan),liver cancer(1179.49yuan),colorectal cancer(1155.98yuan).The results of non-parametric test showed that gender,age,hospital grade,hospital type and length of stay were the influencing factors of direct medical expenses for patients with malignant tumors.The standardized regression coefficients showed that hospitalization days,tertiary hospitals,secondary hospitals and secondary hospitals had a greater impact on the average direct medical expenses.Conclusions The disease burden of patients with malignant tumors in Anhui Province is heavy.Hospital grade,length of stay,hospital type and type of malignant tumors are the main factors affecting the direct medical expenses of patients with malignant tumors at each time.Relevant departments should cooperate to reduce the economic burden of patients with malignant tumors.
