摘 要:目的 通过Meta分析评价超声睫状体成形术(UCP)在治疗青光眼方面的有效性和安全性。方法 检索PubMed、Embase、Cochrane Library、中国知网、维普中文期刊全文数据库、万方数据库和CBM中国生物医学中发表时间在2000年1月至2022年4月的文献,建立纳入和排除标准筛选出符合的文献,详细阅读全文并提取有效数据,应用RevMan(版本V5.3)和Stata(版本16.0)软件对其进行Meta分析。结果 最终纳入10篇文献,共429例患者(含489只眼)纳入研究。Meta分析结果显示,UCP相比睫状体冷冻术(CCP)手术后患者眼压明显较低[MD=-2.43,95%CI(-4.03,-0.82),Z=2.96,P=0.003],而UCP与经巩膜睫状体光凝术(TSCP)手术后患者眼压比较差异无统计学意义[MD=2.96,95%CI(-1.37,7.28),Z=1.34,P=0.18];UCP与其他手术的有效率差异无统计学意义[OR=1.51,95%CI(0.83,2.74),Z=1.34,P=0.18];但UCP术后疼痛程度相比其他术式低[MD=-1.02,95%CI(-1.51,-0.53),Z=4.05,P<0.0001],UCP手术后患者并发症发生率明显少于其他术式[OR=0.26,95%CI(0.17,0.38),Z=6.57,P<0.00001]。结论 UCP治疗青光眼有效且安全,降眼压效果优于CCP,手术有效率与其他手术方式无明显差异,安全性均优于其他手术方式。Objective To evaluate the efficacy and safety of ultrasonic cycloplasty(UCP) to treat of glaucoma by Meta-analysis.Methods From January 2000 to April 2022,literatures published in PubMed, EMBASE,Cochrane library, CNKI,VIP,Wanfang databases and CBM were searched, established inclusion and exclusion criteria were used to screen eligible literatures, read the whole article in detail and extract effective data.RevMan(version V5.3)and Stata(version 16.0)software were used for Meta-analysis.Results Finally, 10 literatures were included into quantitative analysis, and a total of 429 patients(including 489 eyes)participated in the study.Meta analysis showed that compared with Cyclocryotherapy(CCP),UCP had significantly lower intraocular pressure[MD=-2.43,95%CI(-4.03,-0.82),Z=2.96,P=0.003],but no difference in IOP between UCP and Transscleral cyclophotocoagulation(TSCP)[MD=2.96,95%CI(-1.37,7.28),Z=1.34,P=0.18],no difference in the effective rate between UCP and other operations too[OR=1.51,95%CI(0.83,2.74),Z=1.34,P=0.18].However, degree of pain after UCP was lower than that of other methods[MD=-1.02,95%CI(-1.51,-0.53),Z=4.05,P<0.0001].The incidence of complications after UCP was significantly lower than that of other methods[OR=0.26,95%CI(0.17,0.38),Z=6.57,P<0.00001].Conclusion The available data show that UCP could be effective and safe in the treatment of glaucoma, and reducing intraocular pressure would be better than CCP.In terms of surgical efficiency, UCP would have no significant difference compared with other surgical methods, and its safety would be better than other surgical methods.
